摘　　要：目的探讨单光子发射计算机断层/X线计算机断层扫描仪(SPECT/CT)在早期强直性脊柱炎(AS)中的诊断价值。方法选择2013年9月~2016年4月因腰背部疼痛或臀部疼痛就诊于我院的482例患者均符合2009年SPA标准。其中334例符合AS修订版纽约标准,分析SPECT/CT与临床指标的相关性。结果 C-反应蛋白(CRP)与SPECT/CT标志物摄取值有相关性(r=0.279,P=0.045);血沉(ESR)与SPECT/CT标志物摄取值有相关性(r=0.372,P=0.002);ESR与骨侵蚀有相关性(r=0.279,P=0.045);患者疼痛VAS与SPECT/CT标志物摄取值有相关性(r=0.418,P<0.01);患者疼痛VAS与骨侵蚀有相关性(r=0.335,P=0.005)。结论早期AS行SPECT/CT检查,可有效检测早期病变,为尽早诊治提供影像学依据。Objective To investigate the diagnostic value of SPECT/CT in early Ankylosing Spondylitis（AS）.Methods 482 patients with low back pain and hip pain in our hospital from September 2013 to April 2016 received SPECT/CT detection.All patients were fulfilled the standard 2009 ASAS-SPA criteria.Among them,334 patients fulfilled the revised AS New York criteria.The correlation of SPECT/CT and clinical indicators were analyzed.Results CRP had correlation with SPECT/CT markers（r=0.279,P=0.279）.The ESR had correlation with SPECT/CT markers（r=0.372,P=0.002）.The ESR has correlation with bone erosion（r=0.279,P=0.045）.Patients pain VAS had correlation with SPECT/CT markers（r=0.418,P〈0.01）.Patients pain VAS had correlation with bone erosion（r=0.335,P=0.005）.Conclusion Early AS with SPECT/CT examination can effectively detect the early pathological changes and provide imaging basis for early diagnosis and treatment.
