探讨改进基层医院癌痛规范化治疗用药培训方法的研究

摘　　要：目的:分析目前基层医院对癌痛治疗用药的现状及局限性原因,探讨基层医院开展癌痛规范化治疗的用药方法途径。方法:对126例癌痛患者治疗用药情况进行调查,同时对部分临床药师和医生参加癌痛管理培训学习情况调查分析。结果:126例癌痛患者中,98例(77.78%)认为镇痛不足;口服阿片类药物只有38例(30.16%);注射镇痛高达88例(69.84%);被调查30名临床药师及60医师,一年内直接参加癌痛管理培训的临床药师只有6人(20.00%),医师18人(30.00%);其中通过网上学习的临床药师11人(36.67%),医师20人(33.33%);通过阅读杂志文献学习癌痛管理的临床药师10人(33.33%),医师16人(26.67%);行政管理层药师10人,参加面授培训2人(20.00%),网上学习3人(30.00%),阅读杂志文献3人(30.00%),参观学习或上级指导2人(20.00%)。结论:目前基层医院对癌痛治疗用药管理的现状存在不足,对临床药师、医师及相关管理层药师培训有待进一步加强,由于基层存在管理及药物配备等局限性因素,可否通过扩大创建"癌痛规范化治疗示范病房"来改进,值得进一步探讨。Objective Analyze the present situation and limitations of the current treatment of cancer pain in primary hospitals,to explore the ways to standardize the treatment of cancer pain in primary hospitals. Method A survey was conducted on the treatment of 126 patients with cancer pain,and some clinical pharmacists and doctors participated in the study of cancer pain management training. Results Of the 126 cases of cancer pain,98 cases（77. 78%） showed that the analgesia was insufficient,only 38 patients（30. 16%） were treated with oral opioids,as high as 88 cases（69. 84%） with injecting analgesia. Among 30 clinical pharmacists and 60 physicians,there were only 6 clinical pharmacists（20. 00%） and 18 physicians（30. 00%） participated directly in cancer pain management training in one year. And 11 clinical pharmacists（ 36. 67%） and 20 physicians（33. 33%） through online learning,10 clinical pharmacists（33. 33%） and 16 physicians（26. 67%） by reading the magazine literature. Of the 10 executive pharmacist,there were 2 people（20. 00%） attended the face-to-face training,3（30. 00%）through online study,3（30. 00%） by the magazine,and 2（20. 00%） by study or supervisor. Conclusion At present,there are some shortcomings in the management of cancer pain medication in primary hospitals. It is necessary to further strengthen the training of clinical pharmacists,physicians and related executive pharmacist. Due to the existence of management and drug distribution and other limitations in primary hospitals,whether through the expansion of the＂cancer pain standardized treatment model ward＂to improve,it is worth further study.
