不同剂量盐酸氨溴索在老年慢性阻塞性肺疾病急性加重期的应用比较

摘　　要：目的探讨不同剂量盐酸氨溴索在老年慢性阻塞性肺疾病急性加重期(AECOPD)的应用价值。方法 105例老年AECOPD患者随机分为盐酸氨溴索小剂量组(60mg/d)、中剂量组(120mg/d)和大剂量组(300mg/d),每组35例。观察三组患者不同时间段临床总有效率、抗生素使用时间、住院时间及住院费用。结果治疗后第5d的临床总有效率大剂量组要高于中剂量组和小剂量组,差异有统计学意义(均P<0.05);第10d的临床总有效率大剂量组及中剂量组要高于小剂量组,差异有统计学意义(P<0.05);大剂量组抗生素使用时间、住院时间及住院费用要低于中剂量组和小剂量组,差异有统计学意义(均P<0.05)。结论大剂量盐酸氨溴索能提高老年AECOPD患者早期临床疗效,缩短患者抗生素使用时间及住院时间,降低医疗费用。Objective To study the value of different doses of ambroxol in treating acute exacerbation of chronic obstructive pulmonary disease （AECOPD） in elderly patients. Methods Totally 105 acute exacerbation chronic obstructive pulmonary disease patients were randomly divided into small-dose group （60mg/d）; middle-dose group （120mg/d） and high-dose group （300mg/d）; each group has 35 patients. Three groups were observed by clinical efficacy rate, usage period of antibiotics, Length of hospitalization and costs of hospitalization. Results The 5th day after treatment, in clinical efficacy rate, high-dose group is higher than middle-dose group and small-dose group. The difference was statistically significant （all P〈0.05）; The 10th day after treatment, in clini-cal efficacy rate, high-dose group and middle-dose group are higher than small-dose group. The difference was statistically significant （P〈0.05）; for usage period of antibiotics, Length of hospitalization and costs of hospitaliza-tion; high-dose group is lower than middle-dose group and small-dose group, the difference was statistically sig-nificant （all P〈0.05）. Conclusion High-dose of ambroxol can significantly improve the symptoms of AECOPD in elderly patients, short usage period of antibiotics and hospitalization time, reduce hospitalization expense.
