摘 要:目的:了解"在线视频微课堂"方式进行糖尿病教育对中青年2型糖尿病患者自我管理态度和行为的影响。方法:选取2015年于上海市同济医院内分泌科门诊就诊的20~50岁2型糖尿病患者160例,分成试验组(N=80)和对照组(N=80)。试验组通过在线结构化系列微视频进行"在线视频课堂"糖尿病自我管理教育,对照组在就诊时常规进行糖尿病教育。研究前和研究6个月后分别测量两组患者的糖化血红蛋白、空腹血糖和脂代谢指标,糖尿病患者态度量表(DAS-3)和自我管理量表(SDSCA)。结果:研究后试验组患者的糖化血红蛋白下降幅度大于对照组((-0.95±1.54)%比(-0.60±1.51)%,P<0.05);糖尿病管理态度总得分增加幅度均高于对照组((9.75±1.11)比(4.92±2.22),P<0.001);除血糖监测外的自我管理行为得分增加幅度均高于对照组(P均<0.001)。结论 :中青年2型糖尿病患者通过在线视频微课堂形式进行糖尿病自我管理教育,有助于改善糖代谢紊乱、树立正确糖尿病态度、提高糖尿病自我管理能力。Objective: To understand the influence of online video micro class on the self-management attitude and behavior of young and middle-aged patients with type 2 diabetes mellitus. Methods: In 2015, 160 patients with type 2 diabetes mellitus aged 20~50 years old in Department of Endocrinology in Tongji Hospital of Shanghai were selected, and randomly divided into an experimental group(N=80) and a control group(N=80). The experimental group was given an online video micro class for diabetes self-management education through the online structured series micro video, and the control group received routine diabetes education at the time of consultation. Glycated hemoglobin, fasting glucose and lipid metabolism, the diabetic attitudes scale(DAS-3) and the summary of diabetes self-care activities(SDSCA) were measured in the two groups before and after 6 months of study. Results: The decrease of Hb A1 c in the experimental group was greater than that in the control group(-0.95±1.54) % to(-0.60±1.51) %, P〈0.05). The total score of diabetic management attitude was higher in the experimental group than that in the control group(9.75±1.11) to(4.92±2.22), P〈0.001). Except for blood glucose monitoring, the scores of self-management behaviors of the experimental group were all higher than those of the control group(P〈0.001). Conclusion: The education of diabetes self-management by online video micro class in young and middle-aged patients with type 2 diabetes mellitus can help improve glucose metabolism disorders, establish correct attitudes towards diabetes and improve selfmanagement ability of diabetes mellitus.
