摘 要:目的探讨电子纤维支气管镜介入治疗联合常规抗结核治疗方案对支气管结核患者的治疗效果及对肺功能的影响。方法选取2015年1月-2016年1月我院收治的支气管结核患者90例,采用Excel2007生成随机数字表分为联合组、对照组各45例,两组患者均给予常规抗结核方案化疗,联合组同时采用电子纤维支气管镜介入治疗,对比两组治疗效果及肺功能变化。结果治疗后,联合组的CD_3^+、CD_4^+、CD_4^+/CD_8^+值均高于对照组(P<0.05),联合组的CD_8^+低于对照组(P<0.05),联合组的VC、FEV_1、MVC值均高于对照组(P<0.05);治疗后,联合组的临床表现治疗有效率91.11%、胸部CT治疗有效率88.89%均高于对照组的73.33%、68.89%(P<0.05);两组患者的痰菌转阴率比较差异无统计学意义(P>0.05),联合组的痰菌转阴时间平均为2.3±0.7个月,对照组为2.8±0.8个月,联合组的痰菌转阴时间短于对照组(t=2.942,P=0.004)。结论电子纤维支气管镜介入治疗联合常规抗结核治疗方案治疗支气管结核,能显著的改善患者免疫功能、肺功能,提高临床疗效。Objective To investigate the effect of electronic bronchoscope combined with conventional anti tuberculosis therapy in treatment of bronchial tuberculosis. Methods From January 2015 to January 2016 in our hospital,90 cases of bronchial tuberculosis were randomly divided into the combined group and the control group,with 45 cases in each group. All of them were given routine anti tuberculosis chemotherapy,and the combination group was additionally given electronic bronchoscope interventional therapy. The therapeutic effect and changes in lung function were compared between the two groups. Results After treatment,the value of CD_3^+,CD_4^+ and CD_4^+/CD_8^+ was higher in the combination group than in the control group( P < 0. 05),and CD_8^+ was lower in the combination group than in the control group( P < 0. 05). The levels of VC,FEV_1 and MVC were higher in the combination group than in the control group( P < 0. 05). After treatment,the effective rate of clinical manifestations was91. 11%,and the effective rate of chest CT was 88. 89% in the combination group,which were higher than those of the control group( 73. 33% and 68. 89%)( P < 0. 05). There was no significant difference in sputum negative conversion rate between the two groups( P > 0. 05). The mean duration of sputum negative conversion was 2. 3 ± 0. 7 months in the combined group and 2. 8 ± 0. 8 months in the control group. The duration of the sputum was shorter in the combination group than in the control group( t = 2. 942,P = 0. 004). Conclusion Electronic bronchoscope interventional therapy combined with conventional anti tuberculosis treatment can significantly improve the immune function,pulmonary function and clinical efficacy in treatment of bronchial tuberculosis.
