32例淋巴瘘型气管支气管结核的临床分析及早期治疗的探讨

摘　　要：目的回顾性探讨成人淋巴瘘型气管支气管结核的临床特点和早期治疗问题。方法对武汉救治中心2013.01.01-2015.12.31收治的成人淋巴瘘型气管支气管结核32例临床特点、治疗方法、治疗效果、早期诊断等回顾性分析。结果淋巴瘘型气管支气管结核的肉芽结节型及溃疡坏死型形态各异,瘘口可为单个或多个,以单个为多,32例共47个瘘口,主要出现在左右主支气管,右侧为多,单个瘘口组镜下中位治疗次数为5次,2个以上瘘口组中位治疗次数为13次。经药物化疗及镜下介入综合治疗并随访6个月后,淋巴瘘及气道结核基本愈合。结论早期诊断、早期化疗和镜下综合治疗,对疾病的疗效有重要作用。Objective To discuss clinical characteristics and early treatment of adult tracheobronchial tuberculosis induced by lymphatic fistula. Methods The clinical features,treatment,treatment effect and early diagnosis of 32 tracheobronchial tuberculosis induced by lymphatic fistula in Wuhan treatment center from January 2013 to December 2015 were retrospectively analyzed. Results The endoscopic appearance of tracheobronchial tuberculosis induced by lymphatic fistula showed various forms of the granulation nodule and ulcer necrosis. There were 47 orificium fistulae in 32 patients,mainly in the left and right main bronchus,mostly in the right side. The median frequency of treatment was five in patients with single orificium fistulae,whereas the median frequency of treatment was thirteen in patients with multiple fistulae. After chemotherapy and interventional treatment and all patients were followed up for 6 months,lymphatic fistula and airway tuberculosis were healed. Conclusion Early diagnosis,early drug treatment and interventional treatment play an important role in the treatment effect.
