不停泵密闭式双向回血法应用于血液透析结束时的效果观察

摘　　要：目的探讨不停泵密闭式双向回血法应用于血液透析(HD)结束时的临床效果。方法 116例行HD治疗的患者,采用随机数字表法分为对照组与研究组,各58例。对照组采取常规密闭式双向回血法进行HD治疗,研究组应用不停泵密闭式双向回血法进行HD治疗。观察两组回输生理盐水量、回血时间、操作时间、残余液红细胞水平及回血前后心率、舒张压与收缩压水平。结果研究组回输生理盐水量、回血时间、操作时间、残余液红细胞水平分别为(130.50±20.50)ml、(2.82±0.52)min、(3.05±0.52)min、(4865.52±10.50)ml,对照组分别为(152.30±18.40)ml、(3.58±0.68)min、(3.64±0.48)min、(8700.50±12.65)ml;研究组回输生理盐水量、回血时间、操作时间、残余液红细胞水平均优于对照组,差异有统计学意义(P<0.05)。两组回血前后心率、舒张压与收缩压指标组内及组间对比差异均无统计学意义(P>0.05)。结论 HD结束时采用不停泵密闭式双向回血法可有效减少生理盐水回输量,缩短回血时间与操作时间,且对生命体征无影响,适于临床应用。Objective To discuss the clinical effect of non-stop pumping and closed type of two-way blood re-infusion method applied at the end of hemodialysis（HD）. Methods A total of 116 patients treated with HD therapy were divided by random number table method into control group and research group, with 58 cases in each group. The control group was treated with routine closed two-way blood re-infusion method, and the research group was treated with non-stop pumping and closed type of two-way blood re-infusion method. Observation were made on amount of normal saline re-infusion, blood returning time, operation time, residual red blood cell level and the heart rate, diastolic blood pressure and systolic blood pressure before and after blood returning in two groups. Results The research group had amount of normal saline re-infusion, blood returning time, operation time, residual red blood cell level respectively as（130.50±20.50） ml,（2.82±0.52） min,（3.05±0.52） min and（4865.52±10.50） ml, which were（152.30±18.40） ml,（3.58±0.68） min,（3.64±0.48） min and（8700.50±12.65） ml in the control group. The research group had better amount of normal saline, blood returning time, operation time, residual red blood cell level than the control group, and their difference was statistically significant（P〈0.05）. Both groups had no statistically significant difference in heart rate, diastolic blood pressure and systolic blood pressure before and after blood returning within group and between groups（P〉0.05）. Conclusion At the end of HD, non-stop pumping and closed type of two-way blood re-infusion method can effectively reduce the amount of normal saline re-infusion, blood return time and shorten the operation time, and it has no effect on vital signs. It is suitable for clinical application.
