摘 要:目的探讨血清C型利钠多肽(CNP)、胰岛素样生长因子Ⅱ(IGF-Ⅱ)、内皮素(ET)、神经元特异性烯醇化酶(NSE)和S100B蛋白(S100B)对颅脑外伤患者预后的影响。方法选取2016年1月至2017年1月该院收治的颅脑外伤患者110例作为颅脑外伤组,根据格拉斯哥昏迷评分将颅脑外伤组进一步分为轻度、中度和重度颅脑外伤组,采用ELISA法检测所有患者血清CNP、IGF-Ⅱ、ET、NSE和S100B水平,分析其对颅脑外伤患者预后的影响,以及各指标之间的相关性。结果颅脑外伤组入院时血清CNP和IGF-Ⅱ水平明显下降,血清ET、NSE和S100B水平明显上升,与对照组比较差异有统计学意义(P<0.05)。病死组、植物生存组和残疾组血清CNP和IGF-Ⅱ水平明显降低,血清ET、NSE和S100B水平明显增高,与恢复良好组比较差异有统计学意义(P<0.01)。中度和重度颅脑外伤组血清CNP和IGF-Ⅱ水平随病程进展逐渐增加,血清ET、NSE和S100B水平随病程进展逐渐降低,差异有统计学意义(P<0.05)。在颅脑外伤患者中,CNP与IGF-Ⅱ、ET与S100B、ET与NSE、NSE与S100B之间呈现正相关(P<0.01),IGF-Ⅱ与ET、IGF-Ⅱ与S100B、CNP与ET、IGF-Ⅱ与NSE呈明显负相关(P<0.01)。结论血清CNP、IGF-Ⅱ、ET、NSE和S100B与颅脑外伤病变严重程度相关,对颅脑外伤病情判断、预后评估具有较高的临床应用价值。Objective To study the effects of serum C-type natriuretic peptide(CNP),insulin-like growth factor II(IGF-Ⅱ),endothelin(ET),neuron-specific enolase(NSE)and S100 Bprotein(S100 B)on the prognosis of the patients with traumatic brain injury.Methods A total of 110 patients with craniocerebral trauma admitted in our hospital from January 2016 to January 2017 selected as the craniocerebral trauma group and further divided into the mild,moderate and severe craniocerebral trauma groups according to the Glasgow Coma Scale(GCS).Then the levels of serum CNP,IGF-Ⅱ,ET,NSE and S100 Bin all cases were analyzed by enzyme-linked immunosorbent assay(ELISA).Their influence on the prognosis of the patients with craniocerebral trauma and the correlation among various indicators were analyzed.Results The levels of CNP and IGF-Ⅱat admission in the craniocerebral trauma group were significantly decreased,while the levels of ET,NSE and S100 Bwere significantly increased,the difference compared with the control group was statistically significant(P0.05).Serum CNP and IGF-Ⅱlevels in the death group,plant survival group and disabled group were significantly decreased.The difference was statistically significant(P0.01).Serum CNP and IGF-Ⅱlevels in the moderate and severe craniocerebral trauma groups were gradually increased with the disease course progress,while serum ET,NSE and S100 Blevels were gradually decreased with the disease course progress,the difference was statistically significant(P0.05).In the patients with craniocerebral trauma,the positive correlation existed between CNP and IGF-Ⅱ,between ET and S100 B,between ET and NSE,and between NSE and S100 B(P0.01),while the negative correlation existed between IGF-Ⅱand ET,between IGF-Ⅱ and S100 B,between CNP and ET,and between IGF-Ⅱand NSE(P0.01).Conclusion Serum CNP,IGF-Ⅱ,ET,NSE and S100 Bare correlated to the severity of craniocerebral trauma,which has a higher clinical application value for judging the disease condit
