摘 要:目的 探讨国人肩峰指数、肩峰侧倾角及决定性肩角变化的规律及其相关性.方法 回顾性分析2014年6月—2016年6月大连大学附属中山医院和吉林大学第一医院45例肩袖撕裂患者,以及同期50名健康志愿者的肩关节X线影像资料.采用标准肩关节真正前后位X像测量肩峰指数、肩峰侧倾角及决定性肩角,分析两组参数的统计学差异,并对每组内各参数之间进行相关性分析.结果 肩袖撕裂患者的平均肩峰指数为0.71±0.06、平均肩峰侧倾角为78.1°±6.89°、平均决定性肩角为35.9°±5.7°,健康志愿者的平均肩峰指数为0.65±0.06、平均肩峰侧倾角为81.1°±7.74°、平均决定性肩角为32.3°±5.18°,两组差异均有统计学意义(t=4.867、-1.994、3.225,P值均〈0.05).相关性分析显示,肩袖撕裂组和健康对照组研究对象肩峰指数与肩峰侧倾角呈负相关,而与决定性肩角呈正相关(r=-0.873、-0.780,r=0.878、0.924,P值均〈0.01).结论 国人肩峰向侧方的延展度与其向下方的倾斜度呈正相关,与肩胛盂之间的夹角呈负相关.Objective To analyze the variation rule and correlation of acromion index (AI), lateral acromion angle (LAA), and critical shoulder angle (CSA) in Chinese population. Methods The shoulder radiographs of patients with rotator cuff tears (n = 50) and healthy subjects (n = 45) who were admitted to the Affiliated Zhongshan Hospital of Dalian University and the First Hospital of Jilin University from June 2014 to June 2016 were retrospectively reviewed. The standardized true anteroposterior radiography was applied to measure the AI, LAA, and CSA, and the statistical differences of the parameters of the two groups were analyzed, as well as the correlation analysis was carried out between the parameters in each group. Results The mean AI, LAA and CSA was 0. 71 ± 0. 06, 78. 1° ± 6. 89°, 35. 9° ± 5. 7° in patients with rotator cuff tears, respectively, while the corresponding parameters in healthy subjects was 0. 65 ± 0. 06, 81. 1° ± 7. 74°, 32. 3° ± 5. 18°, respectively, and the difference approached statistical significance between the two groups ( t = 4. 867, - 1. 994, 3. 225 respectively, all P values 〈 0. 05). Meanwhile, AI was negatively correlated with LAA, and positively correlated with CSA in both groups (r = - 0. 873, - 0. 780;r = 0. 878,0. 924; all P values 〈 0. 01). Conclusions There was a positive linear relationship between lateral extension and downward slop of the acromion, and a negative linear correlation between lateral extension of the acromion and the angle between the acromion and the glenoid cavity in Chinese population.
