良性阵发性位置性眩晕的认知行为治疗

摘　　要：目的研究认知行为治疗在良性阵发性位置性眩晕(BPPV)治疗中的作用。方法24例确诊为BPPV且焦虑抑郁量表(HADS)评分提示有显著临床精神性症状的患者被随机分为观察组(手法复位+药物治疗+认知行为治疗)和对照组(手法复位+药物治疗),每组12例。在治疗前及治疗后2周分别进行眩晕残障程度评定量表(DHI)、焦虑抑郁量表(HADS)及视觉模拟量表(VAS)评估。结果治疗后,观察组与对照组DHI躯体评分无显著差异(2.00±1.48比2.50±1.51,P>0.05);观察组与对照组DHI情感评分(3.50±1.93比6.00±3.52)、DHI功能评分(6.67±2.74比10.33±3.89)、HADS评分(7.17±1.99比10.58±4.50)及VAS评分(0.58±0.67比1.25±0.87)之差异均具有显著统计学意义,均P<0.05。结论认知行为治疗可改善BPPV患者的情感及功能症状,缓解其焦虑、抑郁情绪,提高生活质量。Objective To investigate the efficacy of cognitive behavioral therapy in the treatment of benign paroxysmal positional vertigo.Methods A total of 24 patients with benign paroxysmal positional vertigo who were suggested having significant clinical psychiatric symptoms by HADS were randomly divided into control group and experimental group,12 in each group.All patients were treated with repositioning maneuvers and additional cognitive behavioral therapy was applied to patients of experimental group.Dizziness Handicap Inventory（DHI）,Hospital Anxiety and Depression Scale（HADS） and Visual Analogue Score（VAS） were performed before and 2 weeks after treatment to assess the efficacy of treatments for patients of two groups.Results After individual treatments,there were significant statistically differences between experimental and control groups in the DHI-E（3.50±1.93 vs 6.00±3.52,P〈0.05）,DHI-F（6.67±2.74 vs 10.33±3.89,P〈0.05）,HADS（7.17±1.99 vs 10.58±4.50,P〈0.05） and VAS scores（0.58±0.67 vs1.25±0.87,P〈0.05）,aside from DHI-P（2.00±1.48 vs 2.50±1.51,P〈0.05）.Conclusion Mental and functional symptoms in patients with BPPV may be improved by cognitive behavioral therapy.The subjective symptoms could be reduced significantly.With solution of anxiety and depression,the quality of life is improved in the patients.
