江苏省张家港市2011-2016年常规免疫接种率监测结果分析

摘　　要：目的了解和评价2011-2016年张家港市常规免疫接种率监测现状,提高免疫规划工作质量。方法对2011-2016年张家港市常规免疫接种率监测数据,采用差值(D)和比值(R)法进行数据分析。结果 2011-2016年张家港市20家预防接种门诊常规免疫接种报表报告完整率100.00%,卡介苗(BCG)、乙肝苗(HepB)、脊髓灰质炎苗(OPV)、百白破苗(DPT)和含麻疹成分疫苗(MCV)报告接种率均>99.00%,而估计接种率为77.65%~133.69%,与报告接种率相差较大。D值评价中,HepB、OPV为可信,BCG、DPT、MCV为可疑。R值评价中,仅OPV/DPT为可信。结论 2011-2016年张家港市常规免疫报告接种率与估计接种率存在差距,主要与应种儿童数掌握方法不一致有关,且与流动儿童免疫规划管理不完善有关。因此,应科学掌握人口数据,加强流动儿童管理,以提高接种质量和监测水平。Objective To explore and evaluate the current situation of routine immunization coverage rates in Zhangjiagang and improve the quality of immunization program. Methods The data of routine immunization coverage rates in Zhangjiagang from 2011 to 2016 were analyzed by using the indexes of difference value （D） and ratio value（R）. Results All 20 vaccination clinics in the city were reported 100. 00% rate of completeness of routine immunization reporting forms. The vaccination rates of Bacillus Calmette Guerin vaccine （BCG）, hepatitis B virus vaccine （HepB）, Poliomyelitis vaccine （OPV）, Diphtheria, Tetanus and Acellular Pertussis Combined vaccine （DPT） and measles containing vaccine （MCV） were reported over 99. 00%. But the esti- mated vaccination rates were 77.65% ~ 133.69%, which were different from the reported vaccination rates. The results of D val- ue evaluation suggested the rates of HepB and OPV credible and the rates BCG, DPT and MCV suspicious. Only the rate of OPV/DPT is credible in R value evaluation. Conclusion There is a significant differences between the reported and estimated rates of routine immunization coverage in Zhangjiagang, which is mainly related to the various methods of counting the number of children and incomplete management of immunization program for floating children. It is necessary to get the population data sci- entifically and strengthen the management of floating children in order to improve the vaccination quality and surveillance level.
