早期综合护理干预在全髋关节置换术后患者尿管管理中的运用

摘　　要：目的:探讨综合护理干预在缩短全髋关节置换术(total hip arthroplasty,THA)后患者尿管停留时间中的运用价值。方法:将2015年7月至2016年2月的43例全髋关节置换患者为对照组;采取常规护理干预措施,将2016年3月至2017年4月的46例患者设为观察组,实施综合护理干预。比较两组术后镇痛泵留置时间、术后疼痛情况、尿管停留时间、排尿功能和排尿主观感受情况。结果:观察组镇痛泵留置时间和尿管停留时间均明显缩短,两组术后各时间点疼痛评分和尿路感染发生率差异均无统计学意义(P>0.05);观察组首次排尿功能和主观感受均明显优于对照组(P<0.05)。结论:综合护理干预方案可有效缩短镇痛泵留置时间和尿管留置时间,促进膀胱功能的恢复,改善患者的排尿感受,值得临床推广。Objective： To investigate the value of comprehensive nursing intervention in shortening the indwelling time of urinary catheter after total hip arthroplasty （THR）. Methods： Forty-three patients with THR treated from July 2015 to February 2016 were selected as a control group, and routine nursing intervention was adopted; 46 patients from March 2016 to April 2017 were selected as an observation group, and comprehensive nursing intervention was adopted. The time of postoperative analgesia pump, the postoperative pain, the indwelling time of the urinary catheter, the urination function and the subjective feeling of urination were compared between the two groups. Results： The indwelling time of analgesia pump and urinary catheter retention time in the observation group shortened significantly; there were no significant differences in pain scores at each time point and urinary tract infection after operation between the two groups （P〉0.05）. The first micturition function and the supervisor feeling of the observation group were better than that of the control group, the difference was statistically significant （P〈0.05）. Conclusion： The comprehensive nursing intervention program can effectively shorten the indwelling time of urinary catheter and indwelling time of the analgesic pump, promote the recovery of bladder function and improve the urination sensation of patients, and is worthy of clinical promotion.
