摘　　要：目的降低肿瘤患者PICC导管内移发生率,提高PICC留置安全性。方法将2016年6月233例患者作为改进前组,行常规PICC导管维护管理;2016年12月249例患者作为改进后组,对发生的PICC导管内移问题进行原因分析,制定改进措施,包括对护理人员进行专项培训,制定导管内移处理流程,在导管固定环节增加贴膜内胶带增强固定度,强化患者健康教育等。结果改进后组PICC导管内移率(12.45%)显著低于改进前组(27.47%,P<0.01),未发生内移相关并发症;改进前组内移患者发生心慌胸闷2例、右心房血栓1例。结论改进管理措施可有效降低PICC化疗患者导管内移,保障患者PICC留置安全。Objective To reduce the incidence of PICC catheter moving for cancer patients,and to improve the safety of PICC indwelling.Methods Totally 233 patients enrolled in June 2016 were considered as before improvement group,which adopted conventional PICC catheter maintenance management.Another 249 patients enrolled in December 2016 were considered as after improvement group,and the reasons of PICC catheter moving were analyzed,the improvement measures were made,which including special training for nurses,draw up the catheter moving disposal process,increase the adhesive tape in the catheter fixation to strengthen the degree of fixation,and intensify health education for patients and so on.Results The incidence of PICC catheter moving of the before improvement group（12.45%）was significantly lower than that of the after improvement group（27.47%,P〈 0.01）,and no moving related complications occurred,while in the after improvement group,2 patients with moving suffered palpitation and chest tightness,and 1 suffered right atrial thrombosis.Conclusion The improvement management can effectively reduce the PICC catheter moving for patients undergoing chemotherapy,and ensure the safety of PICC indwelling.
