生育指数与卵巢巧克力囊肿合并不孕患者术后妊娠率的相关性研究

摘　　要：目的探讨卵巢巧克力囊肿合并不孕患者术后妊娠率与生育指数之间的相关性。方法收集2013年12月-2014年12月73例卵巢巧克力囊肿合并不孕患者的临床资料予以回顾性分析,73例患者均接受手术治疗,对入组患者实施子宫内膜异位症生育指数评分,比较不同评分情况患者术后不同时间的妊娠情况,并进行相关影响因素回归分析。结果对不同子宫内膜异位症生育指数评分患者术后不同时间的妊娠情况进行统计,评分为6、7、8分的患者术后24个月的总妊娠率均显著高于≤5分的患者,评分为7、8分的患者术后24个月的总妊娠率均显著高于6分的患者,差异均有统计学意义(均P<0.05),评分为8分患者的总妊娠率高于7分患者,但经比较差异无统计学意义(P>0.05)。经回归分析,不同子宫内膜异位症生育指数相关指标中,不孕年限评分以及双附件功能均与患者术后妊娠情况之间存在密切关系,差异均有统计学意义(均P<0.05)。结论生育指数与卵巢巧克力囊肿合并不孕患者手术治疗之后的妊娠情况之间关系密切,可以对患者的生育力做出准确的评估,进而为后续治疗提供可靠的参考依据。Objective To explore the correlation between fertility index and pregnancy rate after surgery in patients with ovarian chocolate cyst complicated with infertility. Methods The clinical data of 73 patients with ovarian chocolate cyst complicated with infertility were collected and analyzed retrospectively. All the patients underwent surgical treatment. The data collection time was from December 2013 to December 2014. The patients were divided into two groups,fertility index of endometriosis was given,and the pregnancy rates of patients with different scores and at different time points after surgery were compared. The related influencing factors were analyzed by regression analysis method. Results The total pregnancy rates at 24 months after surgery among the patients with score 6,7,8 were significantly higher than those among the patients with score≤5,the total pregnancy rates at 24 months after surgery among the patients with score 7,8 were significantly higher than that among the patients with score 6,there were statistically significant differences（ all P0. 05）; the total pregnancy rate at 24 months after surgery among the patients with score 8 was higher than that among the patients with score 7,but there was no statistically significant difference（ P 0. 05）. Regression analysis showed that the scores of infertility years and the function of bilateral adnexa were closely correlated with postoperative pregnancy rate（ P 0. 05）. Conclusion Fertility index is closely correlated with postoperative pregnancy rate among the patients with ovarian chocolate cyst complicated with infertility,which can make an accurate assessment of fertility and provide a reliable reference for subsequent treatment.
