血清CEA、LDH对肺腺癌靶向治疗的临床疗效及预后的预测价值

摘　　要：目的探讨血清CEA、LDH对肺腺癌靶向治疗的临床效果与预后的预测作用。方法选取60例接受表皮生长因子受体酪氨酸激酶抑制剂(EGFR-TKI)进行复治的肺腺癌患者为研究对象,测定患者治疗前后血清CEA、LDH水平,并将其水平与影像学标准评估疗效进行比较,观察其预后情况。结果治疗前CEA正常组患者的临床治疗有效率13.3%,治疗前CEA升高组患者的临床治疗有效率37.8%,差异有统计学意义(P<0.05);治疗前LDH正常组患者的临床治疗有效率37.5%,治疗前LHD升高组患者的临床治疗有效率25.0%,差异无统计学意义(P>0.05)。治疗前CEA≥20.0 ng/m L患者PFS为(8.2±1.3)月,CEA<20.0 ng/m L患者PFS为(5.1±1.0)月,2组PFS差异有统计学意义(P<0.05);治疗前CEA≥20.0 ng/m L患者OS为(16.4±2.0)月,CEA<20.0 ng/m L患者OS为(13.1±1.8)月,2组OS差异有统计学意义(P<0.05)。结论治疗前检测血清中CEA水平对预测肺腺癌靶向治疗效果和预后具有重要的作用。Objective To investigate the predictive value of serum CEA ,LDH on clinical efficacy and prognosis of targe-ted therapy for lung cancer .Methods 60 lung cancer patients who underwent the epidermal growth factor receptor tyrosine ki -nase inhibitor （ EGFR-TKI） therapy were selected ,and serum levels of CEA ,LDH were measured ,and compared with radiological criteria assess efficacy to observe prognosis .Results The clinical treatment of patients with normal CEA efficiency was 13.3%, CEA in patients treated with increased clinical efficiency was 37.8%,data comparison between the 2 groups was statistically sig-nificant （ P〈0.05）;the clinical treatment of patients with normal LDH efficiency was 37.5%,LHD elevated group clinical effi-ciency was 25.0%,data comparing the 2 groups showed no significant difference （P〉0.05）.CEA≥20.0 ng/mL in patients with PFS was （8.2 ±1.3） months,CEA〈20.0 ng/mL in patients with PFS was （5.1 ±1.0） months,the PFS difference between the 2 groups was statistically significant （P〈0.05）;OS of patients with CEA≥20.0 ng/mL was（16.4 ±2.0） months,OS of pa-tients with CEA〈20.0 ng/mL was （13.1 ±1.8） months,the 2 groups had statistically significant differences in OS （P〈0.05）. Conclusion Clinical serum CEA to predict lung cancer targeted therapy and prognosis has important role ,combined with LDH detection may increase the overall value .
