植骨融合联合后路经伤椎短节段椎弓根钉固定治疗胸腰段脊椎骨折的临床疗效

摘　　要：目的观察植骨融合联合后路经伤椎短节段椎弓根钉固定治疗胸腰段脊椎骨折的临床效果。方法将64例胸腰段脊椎骨折患者随机分为两组,对照组采用后路经伤椎短节段椎弓根钉固定治疗,观察组采用植骨融合后路经伤椎短节段椎弓根固定治疗。比较两组治疗效果、治疗前、后椎体前缘高度、椎体后缘高度、Cobb角改善情况及脊髓神经功能恢复情况。结果治疗后,观察组优良率达90.6%,显著高于对照组的75.0%(P<0.05)。两组治疗后椎体前缘、后缘高度、Cobb角均显著优于治疗前,且观察组改善效果更显著(P<0.05)。两组治疗后Oswestry功能障碍评分显著低于治疗前,且观察组改善效果更显著(P<0.05)。结论采用植骨融合联合后路经伤椎短节段椎弓根钉固定治疗胸腰段脊椎骨折临床效果显著,椎体前缘高度、椎体后缘高度、Cobb角改善显著,脊髓神经功能恢复良好,值得推广。Objective To observe the clinical efficacy of graft fusion combined with posterior approach of short vertebral pedicle screw fixation in lrealmenl of thoracolumbar vertebral fracture. Methods Sixty -four patients with thoracolumbar vertebral fractures were randomly divided into two groups, the control group adopted posterior approach of short vertebral pedicle screw fixation, while the observation group adopted graft fusion combined with posterior approach of short vertebral pedicle screw fixation. The therapeutic effects, the anterior vertebral height, posterior margin height, improvement of Cobb angle and recovery of spinal nerve function before and after treatment of the two groups were compared. Results After treatment, the excellent rate of observation group was 90.6%, which was significantly higher than 75.0% of the control group （ P 〈0.05）. The anterior vertebral height, posterior margin height and Cobb angle of the two groups were significantly better than those before treatment, and the improvement of the observation group was more significant （ P 〈0.05）. The score of oswestry dysfunction after treatment of the two groups was significantly lower than those before treatment, and the improvement of the observation group was more significant （ P〈0.05）. Conclusion The graft fusion combined with posterior approach of short vertebral pedicle screw fixation has remarkable clinical effect in treatment of thoracolumbar vertebral fracture, the anterior vertebral height, posterior margin height, improvement of Cobb angle and recovery of spinal nerve function can be improved significantly, which is worthy of clinical popularization.
