七氟烷、异氟烷与丙泊酚麻醉对术后疼痛的影响研究

摘　　要：目的探讨七氟烷、异氟烷与丙泊酚在腹腔镜下胆囊切除术患者术后疼痛中的临床效果。方法选择于我院行腹腔镜下胆囊切除手术治疗的患者90例,按照麻醉方式不同将患者分成七氟烷组、异氟烷组及丙泊酚组,各30例。对比分析三组患者的麻醉效果。结果手术前、苏醒时与拔管后,三组患者的舒张压、收缩压及心率比较,无显著差异(P>0.05)。异氟烷组的手术时间明显长于七氟烷组、丙泊酚组(P<0.05)。麻醉后1、2 h,三组患者静息、咳嗽下的NRS评分比较,有显著差异(P<0.05);麻醉后1 h,七氟烷组、异氟烷组患者咳嗽、静息下的NRS评分均高于丙泊酚组(P<0.05);麻醉后2、8 h,七氟烷组、丙泊酚组患者咳嗽、静息下的NRS评分均低于异氟烷组(P<0.05)。结论丙泊酚与七氟烷、异氟烷相比,患者术后的疼痛感比较低,可优先推广应用。Objective To investigate the anesthesia effect of sevoflurane, isoflurane and propofol in postoperative laparoscopic cholecystectomy. Methods Ninety cases of patients with laparoscopic cholecystectomy surgery in our hospital were selected and divided into sevoflurane group, isoflurane group and propofol group according to different anesthesia methods,with 30 cases in each group. Results There were no significant differences on diastolic blood pressure, systolic blood pressure and heart rate among the three groups before surgery, after awakening and extubation（P〈0.05）. The operation time of isoflurane group was significantly longer than that of sevoflurane group and propofol group（P〈0.05）. After anesthesia for1, 2 h, there were significant differences on NRS scores of resting and coughing among the three groups（P〈0.05）. After anesthesia for 1 h, the NRS scores of coughing and resting in the sevoflurane group, isoflurane group were higher than those of the propofol group（P〈0.05）. After anesthesia for 2, 8 h, the NRS scores of coughing and resting in the sevoflurane group and propofol group were lower than those of the isoflurane group（P〈0.05）. Conclusion Compared with sevoflurane and isoflurane, patients with propofol has lower pain after surgery, which can be preferred to promote the application.
