糖皮质激素在结核性脑膜炎治疗中的疗效

摘　　要：目的分析糖皮质激素在结核性脑膜炎治疗中的疗效。方法选择我院2015年12月至2017年1月收治的90例结核性脑膜炎患者并随机分为对照组与研究组,各45例。对照组采用常规疗法,研究组在对照组的基础上给予糖皮质激素治疗。比较两组患者的治疗效果。结果研究组患者结核性脑膜炎治疗总有效率高于对照组(P<0.05);研究组恢复意识时间为(2.51±1.41)d,短于对照组的(5.39±2.77)d(P<0.05);治疗前,两组患者的脑脊液压、白细胞计数、蛋白定量、氯化物浓度比较,差异不显著(P>0.05);治疗后,两组患者的脑脊液压、白细胞计数、蛋白定量、氯化物浓度优于治疗前,且研究组各指标浓度优于对照组(P<0.05)。结论糖皮质激素在结核性脑膜炎治疗中的疗效确切,可有效降低脑脊液压和白细胞计数,改善营养状况,转归良好,值得推广。Objective To analyze the efficacy of glucocorlicoid in the Irealmenl of luberculous meningitis. Methods A lolal of 90 palienls with tuberculous meningitis from December 2015 lo January 2017 in our hospilal were enrolled and randomly divided into control group and study group, with 45 cases in each group. The control group was treated with conventional therapy, and lhe study group was given glucocorlicoid based on lhe control group. The treatment effects of palienls in lhe lwo groups were compared. Results The lolal effective rale of palienls in lhe study group was higher than lhal in lhe control group （ P〈0.05）. The recovery limeofconsciousnessinlhesludygroupwas（2.51±1.41）d,whichwas shorter than （5.39±2.77）dinlheconlrolgroup （ P〈0.05）. Before treatment, lhe cerebrospinal pressure, while blood cell count, protein quantification, concentration of chloride in lhe lwo groups were nol significantly di^erenl （ P〉0.05）; after treatment, lhe cerebrospinal pressure, while blood cell count, protein quantification, concentration of chloride in lhe lwo groups were belter than those before treatment, and those in lhe study group were belter than lhe control group （ P〈0.05）. Conclusion Glucocorlicoid in lhe treatment of luberculous meningitis has exact effect, with can effectively reduce lhe cerebrospinal pressure and while blood cell count, and improve nutritional status with good prognosis, il is worthy of promotion.
