综合护理干预在COPD合并Ⅱ型呼吸衰竭患者无创正压通气治疗中的效果观察

摘　　要：目的观察综合护理干预在慢性阻塞性肺病(COPD)合并Ⅱ型呼吸衰竭患者无创正压通气治疗中的效果。方法选取2010年6月至2014年6月我院诊断治疗的98例COPD合并Ⅱ型呼吸衰竭患者为研究对象,随机分为综合护理组和临床护理组,各49例。临床护理组给予患者平喘、化痰、预防感染等临床常规护理,综合护理组在常规临床护理的基础上给予综合护理干预。观察两组的临床结果。结果护理3 d后,两组PO2及PCO2均明显改善,且综合护理组的上述指标优于临床护理组(P<0.05);综合护理组耐受好发生率高于临床护理组,耐受差发生率明显低于临床护理组(P<0.05)。结论采用综合护理干预护理COPD合并Ⅱ型呼吸衰竭在无创正压通气治疗中临床效果显著,值得临床应用及推广。Objective To invesligale and analyze the effecl of comprehensive nursing inlervenlion on noninvasive positive pressure ventilation in patients of COPD combined with type Ⅱ respiratory failure. Methods A total of 98 patients of COPD with type Ⅱ respiratory failure admitted in our hospilal from June 2010 to June 2014 were selected as the research objects, they were randomly divided into comprehensive nursing group and clinical nursing group, with 49 cases in each group. The clinical nursing group received relieving asthma, reducing phlegm, preventing infection and other routine clinical nursing, the comprehensive nursing group wasgiven comprehensive nursing inlervenlion on the basis of routine clinical nursing. The clinical effects in the two groups were compared. Results After 3 d, the PO2 and PCO2 of the two groups were significantly improved and the those of the comprehensive nursing group was superior to the clinical nursing group （ P〈0.05）; the good tolerance incidence of the comprehensive nursing group was higher than that of the clinical nursing group, the low tolerance incidence of the comprehensive nursing group was significantly lower than that of the clinical nursing group （ P〈0.05）. Conclusion Comprehensive nursing inlervenlion measures has significant effecl on noninvasive positive pressure ventilation in patients of COPD combined with type Ⅱ respiratory failure, which is worthy of clinical application and promotion.
