外科与内镜治疗慢性胰腺炎合并胰管结石的疗效对比

摘　　要：目的探究外科与内镜治疗慢性胰腺炎合并胰管结石的疗效差异。方法选取我院2014年6月至2016年6月收治的慢性胰腺炎合并胰管结石患者84例,其中44例采用外科手术治疗的患者视为对照组,40例采用内镜治疗的患者视为试验组,对两组患者的疗效进行对比分析。结果两组患者的治疗总有效率及结石复发率比较,无显著差异(P>0.05);试验组患者手术时间、术后住院时间、术中出血量均明显低于对照组,差异显著(P<0.05)。对照组患者的二次手术率明显低于试验组,差异显著(P<0.05)。结论在治疗结石残留与临床症状缓解方面,外科与内镜治疗效果相似,但是内镜治疗可以显著缩短手术时间、术后住院时间,降低术中出血量。另外,手术治疗的二次手术概率明显低于内镜治疗。因此,需要根据患者的实际病情合理的选择治疗方法,以便确保患者尽快康复。Objective To explore the differences of the curative effect about surgery and endoscopic treatment in chronic pancreatitis and pancreatic duct stones. Methods A total of 84 patients with chronic pancreatitis and pancreatic duct stones in our hospital from June 2014 to June 2016 were selected, and 44 cases of patients treated with surgical treatment were enrolled in control group, while 40 cases of patients treated with endoscopic were enrolled in experimental group. The curative effects of patients in the two groups were analyzed. Results There were no significant differences in the total effective rate of treatment and recurrence rate of stone between the two groups（P〉0.05）; the operation time, postoperative hospital stay and intraoperative blood loss in the experimental group were significantly lower than those in the control group（P〈0.05）. The secondary operation rate in the control group was significantly lower than that in the experimental group, the difference was significant（P〈0.05）. Conclusion Surgical and endoscopic treatment is similar in the treatment of residual stones and clinical symptoms relief, but endoscopic treatment can significantly shorten the operation time, postoperative hospitalization time, reduce intraoperative blood loss. In addition, the secondary operation rate of the surgical treatment is significantly lower than that of the endoscopic treatment. Therefore, the reasonable treatment should be selected according to the actual condition of patients, to ensure that patients can recover as soon as possible.
