FOCUS-PDCA程序管理方法在提高医务人员手卫生依从率中的应用研究

摘　　要：目的探讨医务人员手卫生管理模式,提高医务人员手卫生的依从性,预防与控制医院感染的发生。方法运用FOCUS-PDCA程序管理方法研究我院手卫生执行过程中存在的不足,分析查找原因并制定解决方案,同时督促落实情况。分析比较干预前后手卫生依从率的变化。结果实施FOCUS-PDCA管理方法后,手卫生依从率从干预前的31.7%提升至69.9%,干预前后差异有统计学意义(P<0.05)。经干预后不同科室、不同类别专业人员及不同手卫生指征的手卫生依从率均有提高,ICU科手卫生依从率提高效果最明显,从干预前的43.3%提升至干预后的85.0%,差异有统计学意义(P<0.05);医技人员手卫生依从率提升最为明显,从干预前的7.3%提升至干预后的52.0%,差异有统计学意义(P<0.05);不同手卫生指征中直接接触患者前提升最明显,从干预前的20.75%提升至干预后的59.08%,差异有统计学意义(P<0.05)。结论 FOCUS-PDCA程序管理方法对提高手卫生依从率效果明显,可改善医务人员手卫生状况,有一定的推广应用价值。Objective To study of hand hygiene of medical staff management, to improve the compliance of hand hygiene of medical staff, the prevention and control of hospital infection.Methods Using the FOCUS-PDCA process as a manage- ment method to find the deficiencies in our hand hygiene management, to collect data, to find the problems, analyze the causes, develop solution, supervise the implementation of the situation, and organize the supervision and inspection com- pare before and after intervention compliance rate of change.Results The implementation of FOCUS-PDCA management method, hand hygiene compliance rate from 31.7% before intervention to 69.9% after the intervention, the differences was significant （P〈0.05） ; The hand hygiene compliance increased more significantly in the department of ICU than in other de- partments, increasing from 43.3% to 85.0% （P〈0.05） ; among all the medical staff the hand hygiene of the medical techni- cians was increased most significantly, increasing from 7.3% to 52.0% （P〈0.05） ; among all the hand hygiene indications, the hand hygiene compliance before contacting a patients was increased most significantly, increasing from 20.75% to 59.08%.Conclusion FOCUS - PDCA process management method can achieve significant effect on improving the hand hygiene compliance, and to ensure the improvement of the medical staff hand hygiene measures are effectively implemen- ted, there is promotional value, hand hygiene management has been continuous improvement.
