摘  要:目的:评估免疫磁珠富集联合多重聚合酶链式反应(multiple polymerase chain reaction,MPCR)技术检测痰液耐甲氧西林金黄色葡萄球菌(methicillin resistant Staphylococcus aureus,MRSA)快速检测及其临床应用。方法:利用免疫磁珠特异性富集金黄色葡萄球菌,联合MPCR技术直接检测痰液标本中MRSA,并以常规细菌培养+Vitek-2全自动微生物分析仪法为标准,评价免疫富集联合MPCR检测的敏感性、特异性。结果:全自动微生物分析仪共在神经外科痰液标本中检出MRSA 21株,总检测率为10.5%,实验耗时48~72 h,检测费用约120元;免疫磁珠富集联合MPCR法共在痰液标本中检出MRSA 24株,检出率12.0%,敏感性100.0%,特异性98.3%,检测耗时降至4~6 h。结论:免疫磁珠联合MPCR法可以直接检测神经外科患者痰液标本中的MRSA,具有敏感、特异、快速等优点,有助于院内MRSA感染的诊断与防控。Objective: To analyze the clinical application value of multiplex polymerase chain reaction(MPCR) combined immunomagnetic method for rapid detection of methicillin resistant Staphylococcus aureus(MRSA) in sputum samples. Methods: Using immunomagnetic enrichment specificity of Staphylococcus aureus, combined with MPCR technique for direct detection of MRSA in sputum, and the conventional bacterial culture Vitek2 automatic microorganism analyzer method as the standard, the sensitivity and specificity of evaluation of immune enrichment combined with MPCR detection. Results: The automatic microorganism analyzer in neurology sputum specimens were detected in MRSA 21 strains, the total detection rate of 10.5%, experiment time 48-72 h, testing costs about 120 yuan; compared with immunomagnetic enrichment combined with MPCR method in sputum specimens were detected in MRSA 24 strains, the detection rate of 12.0%, sensitivity of 100.0%, specificity of 98.3%. Detection time reduced to 4-6 h. Conclusion: The new method can be directly detected in sputum samples of MRSA patients in the Department of Neurosurgery, is sensitive, specific and rapid advantages, has important clini- cal significance for the control of MRSA in the Department and nosocomial outbreaks.
