摘　　要：目的评估AH-1型取石系统(SRS系统)经尿道治疗膀胱结石的疗效和安全性。方法 2008年2月至2017年2月采用自主研发的SRS系统治疗187例膀胱结石。按结石直径分成4组:<2cm组91例,2(2.0~2.9)cm组63例,3(3.0~3.9)cm组21例,≥4cm组12例。回顾性评估不同体积膀胱结石的疗效。结果 187例中95例为单发性结石。碎石时间:<2cm组(9.26±6.53)min,2cm组(14.97±14.50)min,3cm组(20.53±9.43)min,≥4cm组(61.88±45.43)min,4组间差异有统计学意义。125例(73.52%)伴有前列腺增生(BPH)患者同时施行经尿道前列腺电切术(TURP)。TURP手术时间4组间差异无统计学意义。术中无特殊并发症发生。结论采用SRS系统经尿道膀胱碎石可缩短手术时间,降低并发症,扩大经尿道膀胱碎石的手术适应证。可同时实施TURP治疗BPH。是一种安全有效的膀胱结石腔内微创手术方法。Objective To evaluate the safety and efficiency of transurethral cystolitholapaxy with AH-1 Stone Removal System(SRS)in the treatment of bladder stones.Methods Transurethral cystolitholapaxy with SRS was performed in187 patients with bladder stones during Feb.2008 and Feb.2017.The patients were divided into 4 groups according to the stone size:<2 cm group(n=91),2 cm group(2-2.9 cm,n=63),3 cm group(3-3.9 cm,n=21),and≥4 cm group(n=12).The surgical outcomes were retrospectively analyzed.Results Of all 187 cases,95 were single stone,and the stone removal time was(9.26±6.53)min in <2 cm group,(14.97±14.50)min in 2 cm group,(20.53±9.43)min in 3 cm group,and(61.88±45.43)min in≧4 cm group.The difference was statistically significant among the 4 groups.A total of 125(73.52%)patients complicated with benign prostatic hyperplasia(BPH)were treated with transurethral resection of the prostate(TURP)simultaneously.There was no difference in the operation time of TURP among the 4 groups.No significant complication was observed in the surgical procedures.Conclusion Transurethral cystolitholapaxy with SRS can shorten the operation time,reduce the complications, expand the operating indications,and complicated BPH can be effectively treated by TURP simultaneously.Transurethral cystolitholapaxy with SRS is a safe and efficient surgical management for bladder stones.
