地佐辛对瑞芬太尼复合七氟醚全身麻醉患者苏醒期的影响

摘　　要：目的:探讨地佐辛对瑞芬太尼复合七氟醚全麻患者苏醒期的影响。方法:将78例行瑞芬太尼复合七氟醚全麻患者随机分为观察组(39例)和对照组(39例)。对照组和观察组在术毕前30 min分别注射10 ml生理盐水和10 ml用生理盐水稀释的地佐辛(0.10 mg/kg)。检测麻醉前(T0)、拔管时(T1)、拔管后15 min(T2)、30 min(T3)、60 min(T4)的平均动脉压(MAP)、心率(HR)、血氧饱和度(Sp O2)。检测2组的视觉模拟法(VAS)评分和镇静-躁动(SAS)评分。记录2组呼吸恢复时间、呼之睁眼时间、拔管时间和拔管咳嗽评分。结果:在T1、T2、T3和T4时,观察组的MAP和HR的值均明显低于对照组(P<0.05)。在T2、T3和T4时,观察组的VAS评分均明显低于对照组(P<0.05)。在T1和T2时,观察组的SAS评分明显低于对照组(P<0.05)。观察组的拔管咳嗽评分明显低于对照组(P<0.05)。结论:地佐辛可以使瑞芬太尼复合七氟醚全麻患者的血流动力学更加平稳,同时可提高镇痛镇静程度和拔管质量。Objective:To investigate the effect of dezocine on the recovery from general anesthesia induced by remifentanil combined with sevoflurane. Methods Methods:Seventy-eight patients with general anesthesia induced by remifentanil combined with sevoflurane were divided into observation group(39 cases)and control group(39 cases)using random number table method. The observation group were injected with 10 ml dezocine(0.10 mg/kg)and the control group were injected with 10 ml saline solution at 30 min before the end of the operation. The mean arterial pressure(MAP),heart rate(HR)and blood oxygen saturation(Sp O)were2 measured before anesthesia(T0),at extubation(T1),15 min after extubation(T2),30 min after extubation(T3),and 60 min after extubation(T4). The visual analogue scale(VAS)score and Riker sedation agitation scale(SAS)score were detected in the two groups. Respiratory recovery time,the time it takes to open the eyes when hearing the call,extubation time and extubation cough score were recorded. Results Results:The values of MAP and HR in the observation group at T1,T2,T3 and T4 were significantly lower than those in the control group(P<0.05). The VAS scores of the observation group at T2,T3 and T4 were significantly lower than those of the control group(P<0.05). The SAS scores of the observation group at T1 and T2 were significantly lower than those of the control group(P<0.05). The extubation cough score of the observation group was significantly lower than that of the control group(P<0.05).Conclusion Conclusion:Dezocine can facilitate a more stable hemodynamics in patients with general anesthesia induced by remifentanil combined with sevoflurane,can increase the degree of sedation and analgesia,and can increase the quality of pull tube.
