超声在评价老年心力衰竭患者左心形态及功能中的作用

摘　　要：目的探讨超声在评价老年心力衰竭患者左心形态及功能中的作用。方法将128例老年心力衰竭患者作为研究对象,并依据病情将其分为收缩性心力衰竭组(65例)和舒张性心力衰竭组(63例),另选择同时期本院接收的50例健康体检者作为对照组研究对象,回顾性分析3组患者的心脏彩超检查结果,并进行比较。结果收缩组患者左心房内径、左心室内径显著高于对照组和舒张组,而室间隔厚度和左心室后壁厚度显著低于舒张组,差异有统计学意义(P<0.05);舒张组患者左心房内径、室间隔厚度和左心室后壁厚度显著高于对照组,差异有统计学意义(P<0.05);对照组左心室射血分数、二尖瓣舒张流早、晚期速峰值比显著高于收缩组和舒张组,差异有统计学意义(P<0.05);舒张组左心室射血分数显著高于收缩组,差异有统计学意义(P<0.05)。结论对老年心力衰竭患者进行心脏彩超检查能够了解到患者的左心形态及功能,临床应用价值较高。Objective To investigate the role of ultrasonography in evaluation of left ventricular morphology and function in elderly patients with heart failure. Methods A total of 128 patients were enrolled in the elderly patients with heart failure. They were divided into systolic heart failure group(65 cases) and diastolic(63 cases), and 50 healthy subjects were selected as the control group. The results of heart ultrasonography were analyzed retrospectively and compared with the three groups. Results The left atrial diameter and left ventricular diameter were significantly higher in the contraction group than those in the control group and the diastolic group. The thickness of the interventricular septum and the posterior wall of the left ventricle were significantly lower than those of the diastolic group(P<0.05). Thickness and left ventricular posterior wall thickness were significantly higher than the control group(P<0.05); control group left ventricular ejection fraction, mitral valve diastolic flow early and late peak velocity ratio was significantly higher than the contraction group and diastolic group(P<0.05); The left ventricular ejection fraction was significantly higher than that in the shrinking group(P<0.05). Conclusion Cardiac color Doppler ultrasonography can be used to understand the left ventricular shape and function of the elderly patients with heart failure. The clinical application value is high.
