高频电针治疗卒中后尿失禁临床观察

摘　　要：目的观察高频电针治疗卒中后尿失禁的临床疗效。方法将42例卒中后尿失禁患者随机分为2组,高频组22例,低频组20例,2组均取次髎、肾俞穴,选用电针治疗,低频组选用2 Hz,高频组选用50 Hz。2组均连续治疗6次为1个疗程,2个疗程间隔1 d,共治疗4个疗程。观察2组治疗前后排尿间隔时间、夜尿次数、尿失禁程度评分变化,B超测膀胱容量,评定疗效。结果高频组治疗后排尿间隔时间、夜尿次数、尿失禁程度评分及总分均较本组治疗前降低(P<0.05),低频组治疗后排尿间隔时间、尿失禁程度评分及总分均较本组治疗前降低(P<0.05)。高频组治疗后排尿间隔时间、夜尿次数、尿失禁程度评分及总分均低于低频组治疗后(P<0.05)。2组治疗后膀胱容量均较本组治疗前增加(P<0.05),且高频组增加更明显(P<0.05)。高频组总有效率95.45%,低频组总有效率85.00%,2组总有效率比较差异有统计学意义(P<0.05),高频组疗效优于低频组。结论电针治疗卒中后尿失禁效果明显,高频电针能明显改善患者尿失禁程度及膀胱容量。Objective To observe the clinical effects of high-frequency electroacupuncture on the treatment of post-stroke urinary incontinence. Methods 40 patients with post-stroke urinary incontinence were randomly divided into two groups,22 cases in the high-frequency group and 18 cases in the low-frequency group. Electroacupuncture frequency was 2 Hz in the low-frequency group and electroacupuncture frequency was 50 Hz in the high-frequency group were applied to Ciliao( BL 23) and Shenshu( BL 32) respectively. A treatment course was 6 times that continuously treatment for 4 courses,1 d rest between the two courses. The interval of urination,urination frequency at night,the score changes of urinary incontinence and the bladder capacity detected by B ultrasonic before and after treatment were observed,and the effects were evaluated. Results The interval of urination,urination frequency at night,the scores of urinary incontinence and total scores after treatment were reduced in the high-frequency group( P < 0. 05). The interval of urination,the scores of urinary incontinence and total scores after treatment were reduced in the low-frequency group( P < 0. 05). The interval of urination,urination frequency at night,the scores of urinary incontinence and total scores after treatment in the high-frequency group were lower than those in the low-frequency group( P < 0. 05). The bladder capacity after treatment was increased in two groups( P <0. 05),and the increasing in the high-frequency group was more obvious than that in the low-frequency group( P< 0. 05). The total effective rate in the high-frequency group( 95. 45%) was higher than that in the low-frequency group( 85. 0%),the effects in the high-frequency group was superior to that in the low-frequency group( P <0. 05). Conclusion Electroacupuncture treatment has obvious effects on post-stroke incontinence,high-frequency electroacupuncture can significantly improve the degree of urinary incontinence and bladder capacity.
