黄连清心饮加味治疗复发性口腔溃疡的临床疗效

摘　　要：【目的】观察黄连清心饮加味治疗复发性口腔溃疡的临床疗效。【方法】采用随机数字表法将80例患者随机分为治疗组和对照组,每组各40例。治疗组给予口服黄连清心饮加味治疗,对照组给予口服硫酸锌片和复合维生素B片治疗,2组均以4周为1个疗程。观察2组患者局部症状体征的改善情况,评价2组患者的近远期临床疗效。【结果】(1)症状体征积分方面:治疗5 d后,治疗组患者溃疡面积、溃疡疼痛、充血面积、渗出面积、睡眠情况均较治疗前改善(P<0.01),对照组患者仅溃疡面积、充血面积、渗出面积较治疗前改善(P<0.05),治疗组在改善溃疡面积、溃疡疼痛、充血面积、渗出面积、睡眠情况等方面均优于对照组(P<0.01)。(2)近期疗效方面:治疗5 d后,治疗组总有效率为95.0%,对照组为72.5%,治疗组的近期疗效优于对照组(P<0.05)。(3)治疗后1年内,2组溃疡发作总间歇时间均明显延长(P<0.05),总溃疡个数均明显减少(P<0.05),治疗组在延长溃疡发作总间歇时间及减少总溃疡个数方面均优于对照组(P<0.05)。(4)远期疗效方面:治疗后1年内,治疗组总有效率为70.0%,对照组为45.0%,治疗组的远期疗效优于对照组(P<0.05)。(5)治疗过程中,2组患者的血尿常规、肝肾功能及心电图检查,均未发现明显不良反应。【结论】黄连清心饮加味治疗复发性口腔溃疡具有较好的临床疗效,在改善患者的临床症状和体征,延长溃疡发作总间歇时间和减少总溃疡个数方面优于西药治疗。Objective To observe the clinical efficacy of modified Huanglian Qingxin Decoction for the treatment of recurrent oral ulcer. Methods Eighty recurrent oral ulcer patients were randomly divided into treatment group and control group by random number table method,40 cases in each group. The treatment group was given oral use of modified Huanglian Qingxin Decoction,and the control group was treated with Zinc Sulfate Tablets and Compound Vitamin B Tablets. Four weeks constituted one course of treatment. The scores of oral symptoms and signs of the two groups were observed before and after treatment,and the short-term effect and long-term effect of the two groups were evaluated after treatment. Results（1）After treatment for 5 days,ulcer area,ulcer pain,congestion area,exudation area,and sleep of the treatment group were much relieved（P〈0.01）, and ulcer area,congestion area,and exudation area of the control group were improved（P〈0.05）. The treatment group had better effect on improving ulcer area, ulcer pain, congestion area, exudation area, and sleep than the control group（P〈0.01）.（2）After treatment for 5 days, the total short-term effective rate of the treatment group was 95.0%,and that of the control group was 72.5%,the difference being significant（P〈0.05）.（3）Within one year after the suspension of treatment, total intermission period for ulcer attack in the two groups was prolonged（P〈0.05）, and the amount of ulcer foci were much reduced（P〈0.05）, the effect of the treatment group being superior to that of the control group（P〈0.05）.（4） Within one year after the suspension of treatment,the total long-term effective rate of the treatment group was 70.0%,and that of the control group was 45.0%,the difference being significant（P〈0.05）.（5）During the treatment,the results of blood and urine routine examination,hepatic and renal function,and electrocardiography showed no abnormal changes.
