超声造影评价乳腺癌边缘带增殖活性与Ki-67相关性研究

摘　　要：目的应用超声造影技术评价乳腺癌不同感兴趣区灌注情况,并研究Ki-67阳性病灶增强模式及分析癌边缘带灌注参数与Ki-67不同表达的相关性。方法收集乳腺癌患者60例,经超声造影及QLAB软件对不同感兴趣区进行时间-强度曲线分析并进行比较。同时对Ki-67不同表达病灶的增强模式和其与癌边缘带灌注参数进行相关性分析。结果乳腺癌边缘带相对于其他感兴趣区多呈快进慢退、高增强。Ki-67表达阳性乳腺癌病灶多呈快进慢退、高增强,造影剂分布不均匀,造影后病灶范围多数扩大。在研究癌边缘带与Ki-67相关性得出其与峰值强度及曲线下面积呈正相关,与达峰时间呈负相关。结论乳腺癌边缘带为乳腺癌组织增殖最活跃的区域,Ki-67的阳性表达使乳腺癌更具有侵袭性。Objective To evaluate the perfusion of different regions of interest in breast cancer by contrast-enhanced ul- trasonography and to investigate the enhancement pattern of Ki-67 positive lesion and the correlation between the perfusion pa-rameters and the different expression of Ki-67. Methods Sixty patients were collected. Contrast enhanced ultrasonography and QLAB software were used to analyze and compare different regions of interest in the marginal zone by Time intensity curve. At the same time, we analyzed the correlation between the enhancement pattern of Ki-67 expression and the perfusion parameters of the marginal zone of cancer. Results The marginal zone of the cancer was fast forward and slow retreat, high enhancement compared with other regions of interest. Ki-67 positive breast cancer are fast forward slow retreat, high enhance- ment and the distribution of contrast agent were not uniform, The range of lesions enlarged after angiography. The correlation between Ki-67 and marginal zone of the cancer was positive in the peak intensity and the area under the curve and negatively correlated in peak time. Conclusions The marginal zone of breast cancer is the most active region of breast cancer. The posi- tive expression of Ki-67 makes breast cancer more invasive.
