腰椎引流装置在颅内动脉瘤术后患者中的应用及护理

摘　　要：目的探讨动脉瘤术后持续外引流的临床应用及护理对策。方法选取颅内动脉瘤手术患者40例,对照组20例术后进行间断腰穿治疗,观察组20例术后第2天行置管外引流术,促进血性脑脊液廓清,观察2组并发症情况及护理效果。结果观察组颈强直时间、头痛时间分别为(3.6±1.2)、(5.8±2.5)d,短于对照组的(7.2±2.6)、(10.4±1.6)d(P<0.05);观察组脑血管痉挛、脑积水、脑梗死的并发症发生率低于对照组,护理满意度高于对照组(P<0.05);观察组管道留置时间5~14 d,未发生低颅压、颅内感染、管道滑脱等并发症,顺利拔管。结论动脉瘤术后持续外引流可稀释积血,缓解对脑膜的刺激,创伤小,配合正确护理,可保证有效引流,改善预后,提高服务满意度。Objective To analyze the clinical application of lumbar drainage device and nursing countermeasures after the operation of intracranial aneurysms. Methods A total of 40 patients with intracranial aneurysm surgery were selected in this research. The control group( n = 20)received treatment of intercostal lumbar puncture,whereas observation group( n = 20) were treated with external lumbar drainage on the second postoperative day to facilitate the clearance of the hemorrhagic cerebrospinal fluid. The occurrence of complications and nursing of two groups were observed.Results Neck stiffness time and headache time in the observation group were( 3. 6 ± 1. 2),( 5. 8 ±2. 5) d,which were significantly higher than( 7. 2 ± 2. 6),( 10. 4 ± 1. 6) d in the control group( P <0. 05). Complications rates such as cerebral vasospasm,cerebral infarction,and hydrocephalus were remarkably lower,while nursing satisfaction was significantly higher compared with the control group.Catheter indwelling time in the observation group was 5 ~ 14 d,and was smoothly extubated,with no low intracranial pressure,intracranial infection,ductal spondylolisthesis and other complications.Conclusion Continuous external drainage s after the operation of intracranial aneurysms may dilute hemorrhage,relieve meningeal stimulation. And it can guarantee effective drainage,improve prognosis and nursing satisfaction.
