摘 要:军民融合提高血液保障是合作双方以充分发挥各自资源与能力为基础,保障医疗临床用血需求与安全为目的所进行的一系列采供血和血液管理活动。我们主要采取共建献血屋、联合应对突发事件与季节性缺血和偏型与其他临床紧急用血清况,双方紧密配合、互通有无并加强血液管理、积极推广安全血液与自身输血等血液保护新方法和新技术,强调科学合理用血等内容与方式,近10年来,共同有效应对8起重大突发事件的血液保障,防范季节性缺血与临床紧急用血等能力增强,相互调血明显减少,异体输血率下降。2014-2016年,本院每台手术红细胞用量从0.819 8(11 753/14 339)U减少至0.564 9(9 545.5/16 896)U,全院每输血人次拥有红细胞量从0.967 2(11 753/12 151)U升至1.070 4(9 544.5/8 917)U,输血不良反应从0.17%(34/19 979)减少至0.10%(15/14 976)(人次/袋),战备血液储存与军队血液卫勤保障工作也圆满完成。地方血站在整个合作过程中发挥了重要作用,军民融合提高血液保障水平确实可行,并且具有深远的战略意义和十分重要的现实与历史意义。Achieved by the integration of both military and civilian resources,reliable blood supply is a system combining blood collection and management,which aims to further ensure the sfatey in clinical blood application with adequate blood source.The establishment of joint military-civilian blood donation units has been our primary focus,which is designed to cope with unexpected adrupt events,seasonal shortage of blood supply or lack of certain blood types and other clinical emergency blood applications.Close military-civilian co-operation was achieved with frequent exchange of information and materials.Meanwhile,we sought to reinforce blood management by actively promoting new approaches and technologies such as safe blood transfusion and autologous blood transfusion.We also introduced thrombus stretch chart(TEG) detection to our system,emphasizing scientific and rational blood applications.Over the past decade,we joined force and successfully performed blood supply management for 8 major emergencies while maintaining steady improvement coping with seasonal blood shortage and clinical emergency blood applications.We are glad to report a significant reduction,trend over both allogeneic blood transfusion and blood exchanged/transported between agencies due to supply shortage.Reviewing data from 2014 to 2016,we discover that,in our hospital,the amount of erythrocytes comsumed for each operation decreased from 0.819 8 U(11 753/14 339) to 0.564 9 U(9 545.5/16 896); The erythrocyte amount transufsed each dose increased from 0.967 2 U(11 753/12 151) to 1.070 4 U(9 544.5/8 917); Adverse reactions caused by blood transfusion decreased from 0.17%(34/19 979) to 0.10%(15/14 976)(person/bag); War-ready blood storage and military blood medical support were also completed.We hereby conclude that: Local blood banks played an important role in the whole process of cooperation; military-civilian integration to improve blood supply is a practical and feasible appraoch and presents long-term strategic si
