重比重左旋布比卡因单侧腰麻用于老年患者股骨头置换术100例临床观察

摘　　要：目的探讨重比重左旋布比卡因单侧腰麻用于行股骨头置换术老年患者的临床效果。方法将200例行股骨头置换术的老年患者按随机数字表法分为研究组(重比重左旋布比卡因单侧腰麻)与对照组(轻比重左旋布比卡因单侧腰麻),各100例。对比两组血流动力学指标、下肢运动阻滞程度及不良反应发生情况。结果麻醉后10 min及手术结束时,研究组患者的心率(HR)及平均动脉压(MAP)水平均明显高于对照组,Bromage运动阻滞评分明显低于对照组,不良反应发生率明显低于对照组(P<0.05)。结论重比重左旋布比卡因单侧腰麻用于行股骨头置换术老年患者较轻比重左旋布比卡因效果更佳,安全性更好,值得推广。Objective To investigate the clinical effect of unilateral spinal anesthesia with hyperbaric levobupivacaine for treating femoral head replacement for elderly patients. Methods Totally 200 elderly patients underwent femoral head replacement were divided into the study group （unilateral spinal anesthesia with hyperbaric levobupivacaine） and the control group （ unilateral spinal anesthesia with light specific gravity levobupivacaine） according to the random number table method, 100 cases in each group. The hemodynamic indexes,the degree of lower limb motor block and the incidence of adverse reactions were compared between the two groups. Results 10 min after anesthesia and at the end of surgery, the levels of HR and MAR in the study group were significantly higher than those in the control group （ P 〈 0. 05 ） , th e score of Bromage motor block was significantly lower than that in the control group （ P 〈 0. 05 ）, the incidence rate of adverse reactions was significantly lower than that in the control group （ P 〈 0. 05）. Conclusion Compared with unilateral spinal anesthesia with light specific gravity levobupivacaine, unilateral spinal anesthesia with hyperbaric levobupivacaine for treating femoral head replacement for elderly patients has better effect and safety,which is worthy of promotion.
