摘  要:通过拍摄立位X光片来分析慢性非特异性下腰痛(chronic non-specific low back pain,CN LBP)患者腰骶形态结构的改变,评价核心稳定性运动对CN LBP的影响,旨在为CN LBP的临床康复提供参考。方法:招募筛选CNLBP患者23例为实验组(CNLBP组),及29名无腰痛者为对照组(Con组)。CNLBP组自愿参与6周核心稳定性运动,最终完成运动干预共20人。Con组拍摄1次X片;CNLBP组填写评估疼痛程度(VAS)和腰功能障碍(ODI)问卷,并在运动干预前后分别拍摄X光片,测定腰椎前凸程度和骨盆侧倾程度。结果:1)运动前,CNLBP组总腰椎前凸角和骨盆倾斜角与Con组相比有显著差异(P<0.05),腰曲明显变直,骨盆明显侧倾;2)CN LBP组运动前后,VAS和O DI均出现非常显著性下降(P<0.01),下降约50%;3)CNLBP组运动前后,腰椎前凸程度虽未出现显著改变(P>0.05),但数值更接近Con组,腰椎变直的程度有改善的趋势;而骨盆倾斜则发生显著下降(P<0.05)。结论:1)青年CNLBP患者出现明显腰椎变直和骨盆侧倾,此为青年CNLBP的一大特征;2)6周核心稳定性训练可有效改善CNLBP患者的骨盆侧倾程度;3)6周核心稳定性训练可有效改善CNLBP患者的疼痛程度和功能;4)骨盆倾斜角为CNLBP的敏感指标,可为临床诊断和康复疗效评估提供参考。Objective: To investigate the influence of core stability exercise on the Lumbo-sacral structure of chronic non-specific low back pain(CNLBP)with standing X-ray, and provide the reference for the clinical CNLBP rehabilitation. Method: Recruit and screen 23 CNLBP patients as the experimental group(CNLBP Group) and 29 health persons as the control group(Con Group). 20 CNLBP patients finished a 6-week exercise intervention. The Con Group took X-ray film once, while CNLBP tested and recorded the VAS, ODI and X-ray film twice to evaluate the sagittal lumbo-sacral structure(TLL,LSA,SI) and the frontal pelvic position(PI).Results: 1) Before the exercise, there are obvious difference of sagittal lumbo-sacral structure(TLL) and pelvic position(PI) between the CNLBP and Con groups(P〈0.05); 2) After a 6-week exercise intervention, the pain(VAS) and dysfunction(ODI) improved very significantly(P〈0.01); and 3) After a 6-week exercise intervention,the PI of CNLBP improved obviously(P〈0.05), while the lumbar straightness has a trend of improvement.Conclusions: 1) Young CNLBP shows obvious lateral tilt of pelvis, lumbar convex became straight; 2) 6 weeks of core stability exercise can improve the PI effectively, and can improve the straightness of lumbar convex for CNLBP; 3) 6 weeks of core stability exercise can improve the symptom and function; and 4) PI can be seen as a sensitive indicator for CNLBP patients, which can be used for the reference of clinical diagnosis and rehabilitation assessment.
