摘 要:目的研究当归补血汤总苷联合水蛭对大鼠肺纤维化的治疗作用及其机制。方法选取120只雄性SD大鼠,18只作为正常组(生理盐水),其余102只建立肺纤维化模型。72只造模大鼠随机分为模型组(生理盐水)、当归补血汤总苷组(13 mg/kg)、水蛭组(水蛭粉,4 g/kg)、联合组(当归补血汤总苷,13 mg/kg;水蛭粉,4 g/kg),每组18只,灌胃给药。结果造模后第7、14、28天,模型组肺泡炎性评分、肺纤维化程度评分、肺组织中转化生长因子β1(TGF-β1)和纤溶酶原激活物抑制因子-1(PAI-1)、肺泡灌洗液中白细胞介素-4(IL-4)和白介素-17(IL-17)表达水平、肺组织中羟脯氨酸(HYP)含有量均显著高于正常组(P<0.05);当归补血汤总苷组、水蛭组和联合组肺泡炎性评分、肺纤维化程度评分、TGF-β1、PAI-1、IL-4、IL-17表达水平、羟脯氨酸含有量均显著低于模型组(P<0.05);联合组肺泡炎性评分、肺纤维化程度评分、TGF-β1、PAI-1、IL-4、IL-17表达水平、HYP含有量显著低于当归补血汤总苷组、水蛭组(P<0.05)。结论当归补血汤总苷联合水蛭治疗大鼠肺纤维化的效果较好,主要是通过降低肺组织中TGF-β1、PAI-1表达水平、HYP含有量所致。AIM To study the effects of Danggui Buxue Decoction total glycosides combined with Hirudo in the treatment of pulmonary fibrosis in rats and its mechanism. METHODS One hundred and twenty male SD rats were selected,eighteen of which were taken as normal group(normal saline),the remaining 102 rats were used to establish model for pulmonary fibrosis. Seventy-two modeled rats were randomly divided into model group(normal saline),Danggui Buxue Decoction total glycosides group(13 mg/kg),Hirudo group(Hirudo powder,4 g/kg)and combination group(Danggui Buxue Decoction total glycosides,13 mg/kg; Hirudo powder,4 g/kg),eighteen rats in each group with intragastric administration. RESULTS On the 7 th,14 th and 28 th days after modeling,alveolar inflammatory score,pulmonary fibrosis degree score,TGF-β1 and PAI-1 expression levels in lung tissue,IL-4 and IL-17 expression levels in alveolar lavage fluid,hydroxyproline(HYP) content in lung tissue in the model group were significantly higher than those in the normal group(P 〈 0. 05). Alveolar inflammatory score,pulmonary fibrosis degree score,TGF-β1,PAI-1,IL-4,IL-17 expression levels and HYP content in the Danggui Buxue Decoction total glycosides group,Hirudo group and combination group were significantly lower than those in the model group(P 〈 0. 05). In the combination group,alveolar inflammatory score,pulmonary fibrosis degree score,TGF-β1,PAI-1,IL-4,IL-17 expression levels and HYP content were significantly lower than those in the Danggui Buxue Decoction total glycosides group and Hirudo group(P 〈 0. 05). CONCLUSION Danggui Buxue Decoction total glycosides combined with Hirudo have good therapeutic effects on pulmonary fibrosis in rats mainly by reducing TGF-β1,PAI-1 expression levels and HYP content in lung tissue.
