脊髓半横断损伤对大鼠骨折愈合的影响

摘  要:背景:研究证明,神经系统显著影响骨折的愈合过程,神经不同层面、不同成分对骨痂影响各不相同。目的:制备脊髓半横断损伤并骨折的大鼠模型,观察神经完整性不同的骨折,愈合时骨痂量的不同和组织形态学变化,探讨其对骨痂生成的影响。方法:雄性SD大鼠48只,分2组,制备单纯骨折模型及脊髓半横断损伤并骨折模型,切断右半侧脊髓。于术后14,28 d分批取材,取胫骨,称量湿质量,X射线观察骨折愈合情况,苏木精-伊红染色观察骨痂组织形态变化,q PCR SYBR GREEN法检测骨折断端骨组织降钙素基因相关肽、骨形态发生蛋白2 m RNA的表达。结果与结论:(1)脊髓半横断损伤并骨折组,对侧胫骨湿质量大于患侧(P<0.05),均大于单纯骨折组(P<0.05);(2)X射线显示脊髓半横断损伤并骨折,对侧骨痂最大,患侧次之,单纯骨折骨痂最小;(3)苏木精-伊红染色显示,28 d脊髓半横断损伤并骨折组,对侧细胞排列紊乱;患侧纤维骨痂为主,细胞排列较规则;单纯骨折,骨性骨痂为主,细胞排列规则。(4)q PCR SYBR GREEN法结果显示,单纯骨折组降钙素基因相关肽、骨形态发生蛋白2表达较脊髓半横断损伤并骨折组高(P<0.05);(5)结果提示,完整神经支配是骨折愈合的必须因素。BACKGROUND: The nervous system has been proved to play an important role in fracture healing, and different nervous components exert different effects on bony callus.OBJECTIVE: To observe the callus volume and morphological changes in the fracture healing progress of the rats withspinal cord hemisection and fracture.METHODS: Forty-eight male Sprague-Dawley rats were divided into two groups, and the models of simple fracture andspinal cord hemisection by cutting the right spinal cord combined with fracture were established. The tibia was removedat 14 and 28 days after surgery, and the wet weight was measured, the fracture healing was observed by X-rayexamination, morphological changes of the callus were observed through hematoxylin-eosin staining, and the expressionlevel of calcitonin gene-related peptide mRNA in the broken bones was detected by SYBR Green qPCR.RESULTS AND CONCLUSION: In the combination group, the wet weight of the contralateral tibia was more than that ofthe affected side (P 〈 0.05), which both were more than those in the simple group (P 〈 0.05). X-ray findings showed thatthe combination group had the largest lateral callus, followed by the affected side, and the smallest in the simple group.Hematoxylin-eosin staining showed that in the combination group, the contralateral cells arranged in disorder, while thefibrous callus and cells in regular arrangement were found in the affected side; there were bony callus and cells in regulararrangement in the simple group at 28 days after surgery. SYBR Green qPCR results revealed that the expression levelof calcitonin gene-related peptide mRNA in the simple group was significantly higher than that in the combination group(P 〈 0.05).
