甘精胰岛素在初发2型糖尿病治疗中的应用

摘  要:目的研究甘精胰岛素治疗初发2型糖尿病(Type 2 diabetes mellitus,T2DM)的临床疗效。方法选择2014年12月-2016年11月在我院接受治疗的初发T2DM患者96例,依据随机数字表法分为甘精胰岛素组(n=48)与对照组(n=48)。两组患者均给予胰岛素强化治疗及健康教育、运动及饮食控制等综合治疗。在上述治疗基础上,甘精胰岛素组给予甘精胰岛素治疗,对照组给予精蛋白锌胰岛素治疗。观察比较两组治疗前及治疗后HBA1C、FBG及PBG水平,空腹C肽及餐后2 h C肽水平,胰岛素敏感指数(ISI)及胰岛素抵抗指数(IRS),治疗期内低血糖发生情况。结果治疗后,两组HBA1C、FBG及PBG水平均降低,甘精胰岛素组HBA1C、FBG及PBG水平均低于对照组,差异有统计学意义(P〈0.05)。两组空腹C肽及餐后2h C肽水平均增加,甘精胰岛素组空腹C肽及餐后2h C肽水平均高于对照组,差异有统计学意义(P〈0.05)。两组ISI均增大,IRS均减小,甘精胰岛素组ISI大于对照组,IRS小于对照组,差异有统计学意义(P〈0.05)。治疗期内,甘精胰岛素组低血糖发生率(6.25%)低于对照组(25.00%),差异有统计学意义(P〈0.05)。结论精胰岛素治疗初发T2DM可有效降低HBA1C、FBG及PBG水平,提高C肽水平,提高胰岛素的敏感性,降低胰岛素抵抗,低血糖发生率低,值得临床应用。Objective To study the clinical efficacy of glycine insulin in treating primary Type 2 diabetes mellitus(T2DM). Methods 96 cases of T2 DM patients were treated in our hospital from December 2014 to November 2016, according to the random number table method, which was divided into two groups(n = 48) and control group(n = 48). Both groups were given comprehensive treatment of insulin intensive therapy and health education, exercise and diet control. On the basis of the above treatment, glycine insulin group was given insulin treatment, and the control group was given zinc insulin-insulin treatment. Compare two groups before and after the treatment level of HBA1 C, FBG, PBG, fasting C peptide and postprandial 2 h C peptide levels and insulin sensitive index(ISI) and insulin resistance index(IRS), the occurrence of hypoglycaemia treatment period. Results After treatment, both groups of HBA1 C, FBG and PBG were lower, and the levels of HBA1 C, FBG and PBG in glycine insulin group were lower than those in the control group, and the difference was statistically significant(P < 0.05). The levels of the two groups of fasting C peptide and 2h C peptide were all increased, and the levels of fasting C peptide and 2h C peptide of glycine insulin group were higher than those in the control group, and the difference was statistically significant(P < 0.05). Both groups increased and the IRS decreased. The ISI was larger than the control group, and the IRS was smaller than the control group, and the difference was statistically significant(P < 0.05). During the treatment period, the incidence of hypoglycemia(6.25%) was lower than the control group(25.00%), and the difference was statistically significant(P < 0.05). Conclusion Insulin treatment of incipient T2 DM can effectively reduce the level of HBA1 C, FBG, PBG, improve the level of c-peptide, improve insulin sensitivity, reduce insulin resistance, lower incidence of hypoglycemia, worthy of clinical application.
