针刺联合补阳还五汤治疗气虚血瘀证冠心病稳定型心绞痛疗效及对血脂水平的影响

摘　　要：目的观察针刺联合补阳还五汤治疗气虚血瘀证冠心病稳定型心绞痛患者的临床疗效及对血脂水平的影响。方法将符合纳入标准的70例气虚血瘀证冠心病稳定型心绞痛患者随机分为治疗组和对照组,每组35例。对照组给予冠心病二级预防药物规范化治疗,治疗组在对照组治疗基础上给予针刺联合补阳还五汤口服,2组均以4周为1个疗程。治疗1个疗程后,统计2组心绞痛疗效、心电图疗效、硝酸甘油停减疗效,并比较2组治疗前后中医证候积分及血脂水平的差异。结果治疗组心绞痛疗效、硝酸甘油停减疗效均明显优于对照组(P均<0.05),2组心电图疗效相比差异无统计学意义(P>0.05);治疗后2组中医证候积分均明显降低(P均<0.05),血脂指标均明显改善(P均<0.05),且治疗组中医证候积分和TG、LDL-C水平均明显低于对照组(P均<0.05)。结论针刺联合补阳还五汤可有效缓解气虚血瘀证冠心病稳定型心绞痛患者的临床症状,改善心电图缺血征象,减少患者硝酸甘油使用量,降低TG、LDL-C水平。Objective It is to observe the clinical effect of acupuncture combined with Buyang Huanwu Decoction on stable angina pectoris with syndrome of Qi deficiency and blood stasis and on the levels of blood lipid. Methods 70 patients with sta- ble angina pectoris with syndrome of Qi deficiency and blood stasis were randomly divided into treatment group and control group, each group had 35 cases. Both groups were treated with conventional therapy with prevention drugs for coronary heart disease. The treatment group were additionally treated with acupuncture combined with Buyang Huanwu Decoction. 4 weeks as one course, after treatment for one course, the curative effects of angina pectoris, ECG, nitroglycerin stopping rate were calcu- lated, and the differences in TCM syndromes and blood lipid were observed and analyzed between the two groups. Results The total effective rate of angina pectoris, nitroglycerin stopping rate in the treatment group were significantly higher than those in the control group（P 〈 0.05）. There was no significant difference in total effective rate of ECG between the two groups （ P 〉 0.05）. After treatment, the scores of syndrome of TCM were significantly decreased, the indexes of blood liquid were signifi- cantly improved in both groups （ P 〈 0.05 ） , and the scores and the levels of TG, LDL-C after treatment in the treatment group were lower than those in the control group （ P 〈 0.05 ）. Conclusion Acupuncture combined with Buyang Huanwu Decoction could effectively relieve clinical symptoms, improve ECG signs of ischemia, decrease the dose of nitroglycerin and TG, LDL- C in the patients with stable angina pectoris with syndrome of Qi deficiency and blood stasis.
