清心舒肝法联合妥泰治疗小儿多发性抽动症疗效观察

摘　　要：目的观察清心舒肝法联合妥泰治疗小儿多发性抽动症(MT)的疗效。方法将80例MT患儿随机分为2组,每组40例,对照组给予妥泰0.5~2 mg/(kg·d)口服,研究组给予妥泰联合清心舒肝汤、电针、耳穴压豆等清心舒肝法治疗,持续治疗3个月,观察治疗前、后中医证候积分变化,采用耶鲁综合抽动严重程度量表(YGTSS)评价抽动严重程度变化,并检测血浆神经递质变化。结果研究组治疗后呓声秽语、多梦易醒、烦躁易怒、皱眉眨眼、张口歪嘴、摇头耸肩证候积分均显著低于对照组(P均<0.05);研究组治疗后YGTSS评分减分幅度均显著大于对照组(P均<0.05);治疗后研究组多巴胺、5-羟色胺、5-羟吲哚乙酸等神经递质含量均显著低于对照组(P均<0.05)。结论清心舒肝法联合妥泰治疗小儿MT可缓解中医证候,减轻甚至消除抽动症状,这可能与清心舒肝法影响患儿血浆神经递质有关。Objective It is to observe the therapeutic effect of therapy of clearing heart and soothing liver combined with topiramate in the treatment of children with multipletics （ MT）. Methods 80 children with MT were divided into two groups by random number table method, 40 children in each group. The control group was treated with topiramate 0.5 -2.0 mg/（ kg · d） , while the study group was treated with decoction for clearing heart and soothing liver, electric acupuncture and ear-point therapy, both groups were continuously treated for 3 months. The changes of TCM syndrome scores were observed, and twitch severity was evaluated by Yale Global Tic Severity Scale （YGTSS） , and plasma neurotransmitter was detected before and after treatment in both groups. Results After treatment, the scores of tourette syndrome, deaminess and festless sleep, irritability, making eyes, mouth and head shook in the study group were significantly lower while decrease of YGTSS scores were more than those in the control group （P 〈 0.05） , the contents of dopamine, serotonin and 5-hydroxy indole acetic acid neurotransmitter in the study group were lower than those in the control group （P 〈 0.05）. Conclusion Therapy of clearing heart and soothing liver combined with topiramate can alleviate the syndromes of TCM, reduce or even eliminate twitch symptoms in the treatment of child MT, this may be related to its influence on plasma neurotransmitter involved in the children.
