摘 要:目的探讨青藏高原藏族人群Graves病(GD)与细胞毒性T淋巴细胞相关抗原(CTLA-4)基因多态性及炎症因子的相关性。方法以96例世居于海拔3 000米以上的GD患者为研究对象纳入观察组,选择同期90例世居于海拔3 000米以上的健康体检者纳入对照组A,90例世居于海拔2 200米健康体检者纳入对照组B,对三组对受试者CTLA-4基因多态性及血清肿瘤坏死因子(TNF-α)、白细胞介素-6(IL-6)水平与三碘甲状腺原氨酸(FT_3)、游离甲状腺素(FT_4)水平的相关性进行观察。结果观察组CTLA-4基因1+49 A/G位点G/G基因型频率及G等位基因频率高于对照组A及B,A/A基因型频率及A等位基因频率明显低于对照组A和对照组B(P<0.05);IL-6及TNF-α水平均与FT_3、FT_4呈正相关(P<0.05)。结论CTLA-4外显子1+49位点的多态性可能与青海藏族GD患者疾病的发生存在一定相关,炎性因子水平检测可能对GD病情的诊断及治疗效果的判断有一定指导意义。Objective To observe the correlative relationship between Graves disease(GD)and CTLA-4 gene polymorphism and inflammatory factors in Tibetan population of Qinghai-Tibet plateau.Methods The study group:96 cases of GD patients over 3 000 meters above sea level over3 000 meters above sea level;The control group A:90 cases of high altitude Tibetan health volunteers on 2 200 meters above sea level;The control group B:90 cases of low altitude healthy volunteers.Three groups of CTLA-4 gene polymorphisms and the correlation between TNF-αand IL-6 levels and FT_3and FT_4 levels were observed.Results The frequencies of G/G genotype and G allele of CTLA-4 gene 1+49 in observation group were significantly higher than control group A and B,A/A genotype frequency and A allele frequency were significantly lower than the control group A and B(P<0.05);IL-6 and TNF-αlevels were positively correlated with FT_3 and FT_4(P<0.05).Conclusion Polymorphism of exon 1+49 locus of CTLA-4 may be related to the occurrence of disease in Qinghai Tibetan GD patients,the detection of inflammatory factor may be helpful in the diagnosis and treatment of GD disease.
