摘 要:目的分析探讨舒芬太尼与芬太尼在用于神经外科手术麻醉中的麻醉效果以及对患者麻醉恢复情况、血流动力学以及机体应激反应的影响。方法随机选取我院神经外科于2015~2016年期间实施择期开颅手术的患者66例,等分为对照组和观察组两组,分别应用芬太尼和舒芬太尼,同时复合咪唑安定、维库涅安以及异丙酚进行麻醉诱导,复合七氟烷进行麻醉维持。结果两组患者在麻醉诱导后,心率(HR)、收缩压(SBP)以及舒张压(DBP)均较麻醉前明显下降(P<0.05)。拔管后,对照组患者血流动力变化明显大于对照组患者(P<0.05),同时两组患者在苏醒期睁眼、呼吸恢复以及拔管时间等观察指标方面也有着明显的差异性,P<0.05,具有统计学意义。结论在神经外科手术麻醉过程中应用舒芬太尼,患者在不同麻醉时期的血流动力学均较为平稳,术后拔管时间短,意识恢复快,进而有利于对患者进行术后镇痛管理和呼吸道管理,同时还能够有效的缓解全麻诱导时行器官插管对心血管系统造成的不良反应,因此相对于芬太尼而言,舒芬太尼更加适宜用于神经外科临床手术麻醉中。Objective To investigate the effect of sufentanil and fentanyl in anesthesia for Department of Neurosurgery surgery anesthesia and effect on anesthesia recovery,hemodynamics and stress reaction.Methods 66 patients were randomly selected from the Department of neurosurgery in our hospital during 2015 -2016 undergoing elective craniotomy,divided into control group and observation group of two groups application of fentanyl and sufentanil, respectively, at the same time, Vikune Ann and propofol combined with midazolam anesthesia, sevoflurane anesthesia combined with seven maintenance.Results Two groups of patients after anesthesia induction(HR),heart rate, systolic blood pressure(SBP) and diastolic blood pressure(DBP) were compared with those before anesthesia was significantly decreased(P〈0.05).After extubation,patients in the control group. The hemodynamic changes were significantly higher than control group(P〈0.05), while two patients in the recovery period of eyes, breathing recovery and extubation time in the observation index also has the obvious difference(P〈0.05),with statistical significance.Conclusion Application of sufentanil anesthesia in Department of neurosurgery in the process of hemodynamics in patients with different anesthesia period was relatively stable,postoperative extubation time,consciousness recovery, and is beneficial to patients with postoperative pain management and the management of respiratory tract, but also can effectively alleviate the adverse reaction during induction of general anesthesia intubation caused organ on the cardiovascular system, so relative to fentanyl speaking,more suitable for clinical surgery anesthesia with sufentanil in Department of neurosurgery.
