宫颈环形电切术联合康复灵胶囊治疗宫颈上皮内瘤变伴高危型HPV感染60例

摘　　要：目的:观察宫颈环形电切术(loop electrosurgical excision procedure,LEEP)联合康复灵胶囊治疗宫颈上皮内瘤变(cervical intraepithelial neoplasia,CIN)伴高危型人乳头状病毒(human papilloma virus,HPV)感染的临床疗效。方法:将120例CIN合并HPV患者随机分为对照组和观察组,每组各60例。对照组采用LEEP术治疗,观察组在LEEP术的基础上联合康复灵胶囊治疗。观察两组患者的临床疗效、术后阴道出血、阴道排液、宫颈创面愈合时间、CIN治愈率及HPV转阴率,比较两组患者治疗前后临床症状积分变化情况。结果:两组患者治疗后临床症状积分较治疗前下降,且观察组低于对照组,差异具有统计学意义(P<0.05)。对照组有效率为78.33%,观察组有效率为96.67%,两组患者临床疗效比较,差异具有统计学意义(P<0.05)。观察组阴道出血量及阴道排液量小于对照组,阴道出血时间、阴道排液时间及宫颈创面愈合时间均短于对照组,差异均有统计学意义(P<0.05)。观察组术后3个月、6个月CIN治愈率分别为90.00%、96.6%,显著性高于对照组的71.7%和83.33%,差异均有统计学意义(P<0.05);观察组术后3个月、6个月HPV转阴率分别为63.33%、81.67%,对照组的HPV转阴率分别为33.33%、46.7%,两组比较,差异有统计学意义(P<0.05)。结论:LEEP术联合康复灵胶囊治疗可显著改善宫颈上皮内瘤变伴高危型HPV感染患者术后临床症状,提高CIN治愈率和HPV转阴率。Objective： To observe the clinical efficacy of loop electrosurgical excision procedure（ LEEP） combined with Kangfuling Capsules on cervical intraepithelial neoplasia（ CIN） with clinical high-risk HPV infection. Methods： Totally 120 cases of CIN patients treated in our hospital between January 2016 and January 2017 were divided into control group and observation group,60 cases in each group. Control group were given LEEP treatment,while the observation group were additionally given Kangfuling Capsules. Results： After treatment,the TCM symptom scores in both groups significantly decreased（ P〈0. 05）,and observation group decreased more significantly（ P〈0. 05）. The effective rate was 96. 67% in observation group and 78. 33% in control group（ P〈0. 05）. The vaginal bleeding volume and bleeding time,vaginal discharge volume and time,and the healing time of cervix in control group were significantly more than those in observation group（ P〈0. 05）. After 3 months and 6 months of operation,the cure rate of CIN was 90. 00% and 96. 6% in observation group,higher than 71. 7% and 83. 33% in control group（ P〈0. 05）; the negative conversion rate of HPV were 63. 33% and 81. 67% respectively in observation group,significantly higher than 33. 33% and 46. 7% in control group（ P〈0. 05）. Conclusions： LEEP combined with Kangfuling Capsules can significantly improve the clinical symptoms of CIN patients with high-risk HPV infection,which can improve the cure rate of CIN and the negative conversion rate of HPV.
