摘 要:目的研究100 kVp条件下、碘克沙醇(270 mgI/ml)应用于肾动脉CTA的可行性。方法回顾性分析行肾动脉CTA检查的40例患者资料,分为两组,分别采用不同CT扫描及对比剂注射方案:双低组30例,管电压100 kVp,对比剂碘克沙醇270 mgI/ml;常规组:管电压120 kVp,对比剂碘佛醇320mgI/ml。比较两组临床资料,图像质量主、客观评价指标、CT辐射剂量及对比剂用量。结果两组患者年龄及体重指数间差异无统计学意义(P=0.661,P=0.641)。图像质量主观评价及分支显示差异无统计学意义(P=0.312,0.588),图像质量客观评价双低组肾动脉CT值及背景噪声高于常规组(P<0.001),竖脊肌CT值、肾动脉CNR及SNR两组间差异无统计学意义(P=0.206,0.859,0.65)。双低组CT辐射剂量及应用对比剂碘总量低于常规组(P<0.001,P=0.006)。结论100 kVp条件下、碘克沙醇(270 mgI/ml)应用于肾动脉CTA图像质量满足诊断要求,且较常规方案降低了对患者的危害,推荐作为临床常规扫描方案。Objective To investigate the feasibility of the 100 kVp protocol using 270 mgI/kg contrast medium for CTA of renal arteries.Methods 40 patients underwent renal artery CTA and they were divided into two groups (Group A and B) based on different scan protocols.Group A included 30 patients using the following parameters:100 kVp and iodixanol (270 mgl/ml).Group B included 10 patients using following parameters:120 kVp and ioversol(320 mgI/ml).The clinical information,objective and subjective assessment of image quality,CT radiation and contrast medium dose of these two groups were retrospectively analyzed.Results The difference of age and BMI of two groups was not statistically significant (P =0.661,0.641).The difference of subjective assessment score of image quality in the two groups was not statistically significant (P =0.312,0.588).The CT values of renal artery and image noise were higher in group A,with significant statistic difference (P 〈 0.001).The difference of CT values of erector spinae,CNR and SNR of renal artery in two groups was not statistically significant(P =0.206,0.859,0.65).The dose of CT radiation and contrast medium was lower in group A,with significant statistic difference (P 〈 0.001,P =0.006).Conclusion CTA of renal arteries using tube voltage of 100kVp and low-concentration contrast media of 270 mgI/ml reduces radiation dose and iodine load,and its objective image quality and subjective measurements are acceptable for diagnostic function.
