肺炎性肌纤维母细胞瘤的CT影像表现及病理对照

摘　　要：目的探讨肺部炎性肌纤维母细胞瘤(IMT)的CT表现及诊断价值。方法回顾性分析10例经穿刺或手术病理证实的肺部IMT的CT表现及临床表现,9例行CT平扫及增强扫描,1例行CT平扫。结果 10例病灶均为单发。9例位于右肺,其中8例位于肺外带,表现为不规则肿块,4例可见"晕征";1例位于左肺,表现为实性小结节影,境界清晰。2例病灶合并邻近肺组织不张,4例病灶可见不同程度的囊变、坏死,4例病灶与邻近胸膜黏连。1例病灶中可见点状钙化,1例病灶可见"脐凹征"。7例表现为不均匀强化,1例表现为均匀强化。结论肺IMT的CT表现多种多样,无明显特异性,结合临床病史、CT影像学表现及实验室检查全面分析有助于提高肺IMT诊断的准确性。Objective To explore the CT features of inflammatory myofibroblastic tumor（IMT） and their diagnostic value.Methods The CT image features of 10 cases with IMT confirmed by surgery or needle biopsy and pathological examination were retrospectively analyzed.9 cases were performed with plain and enhanced scanned,1 case had plain scan only.Results The 10 lesions were single.9 were located in the right lung,among which,8 were located in the periphery of the lung presented as irregular masses,halo sign appeared in 4 cases;1 presented as a solid nodule with well-defined margin;2 cases showed atectasis;cystic changes and necrosis were found in 4 cases;adjacent pleural adhesion were found in 4 cases.Dot-like calcification was found in 1 case;dent depression sign was found in 1 case.7 cases showed heterogeneous enhancement,while 1 case showed homogeneous enhancement.Conclusion The CT features of IMT are variable and have no obvious characteristics.Comprehensive analysis of CT image features,clinical history and laboratory tests will improve the diagnosis accuracy.
