右美托咪定对腹腔镜子宫肌瘤切除术麻醉诱导期镇痛的影响

摘　　要：目的探讨右美托咪定对腹腔镜子宫肌瘤切除术麻醉诱导期镇痛的影响。方法 96例腹腔镜子宫肌瘤切除术患者,按用药不同将患者分为对照组和观察组,各48例。对照组患者给予生理盐水联合舒芬太尼,观察组患者给予右美托咪定联合舒芬太尼。比较两组患者各时间点Narcotrend指数(NTI)及拔管后20 min(T3)~苏醒后(T4)时NTI指数降低至最低值的时间等。结果观察组患者拔管前(T2)时NTI指数为(83.19±13.00),低于对照组的(89.09±9.68),差异有统计学意义(P<0.05);两组患者麻醉诱导前(T1)、T3、T4时NTI指数比较差异无统计学意义(P>0.05)。观察组在T3~T4时NTI指数降低至最低值的时间为(221.85±74.60)s,短于对照组的(257.46±54.73)s,差异有统计学意义(P<0.05)。两组患者NTI指数最低值比较差异无统计学意义(P>0.05)。结论右美托咪定可有效提高对腹腔镜子宫肌瘤切除术患者在麻醉诱导期舒芬太尼的镇痛效果。Objective To investigate the effect of dexmedetomidine on analgesia during anesthesia induction for laparoscopic myomectomy. Methods A total of 96 patients undergoing laparoscopic myomectomy were divided by different meditations into control group and observation group, with 48 cases in each group2 The control group received normal saline combined with sufentanil, and the observation ~roup received dexmedetomidine combined with sufentanil. Comparison were made on Narcotrend index （NTI） and time of 20 min after extubation （T3）after awakening （T4） NTI index to minimum at all time points between two groups. Results The observation group had lower NTI index at before extubation （T2） as （83.19 ± 13.00） than （89.09 ± 9.68） in the control group, and the difference was statistically significant （P〈0.05）. Both groups had no statistically significant difference in NTI index at before anesthesia induction （T1）, T3 and T4 （P〉0.05）. The observation group had time of T3-T4 NTI index to minimum as （221.85 ± 74.60） s, which was shorter than （257.46 ± 54.73） s in the control group, and the difference was statistically significant （P〈0.05）. Both groups had no statistically significant difference in NTI index minimum （P〉0.05）. Conclusion Dexmedetomidine can effectively improve the analgesic effect of sufentanil during anesthesia induction for laparoscopic myomectomy.
