摘 要:目的观察黄连煎剂对果糖诱导的代谢综合征大鼠代谢指标和肾脏的保护作用。方法雄性Wistar大鼠56只,随机选取6只大鼠作为正常对照组,正常饮水。其余50只大鼠每日饮用10%果糖水,连续8周建立代谢综合征模型,24只大鼠造模成功后随机分为模型组、黄连煎剂组、盐酸小檗碱组及牛磺酸熊脱氧胆酸(TUDCA)组。比较各组大鼠体重、内脏脂肪重量(VFW)、空腹血糖(FBG)、空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)、胆固醇(TC)、甘油三酯(TG)、收缩压(SBP)、舒张压(DBP)、尿N-乙酰-β-D-葡萄糖苷酶(NAG)、尿微量白蛋白(m-ALB)、尿素氮(BUN)及血肌酐(Scr)水平,光镜及电镜下观察肾脏病理改变。结果造模后,模型组体重、VFW、FINS、HOMA-IR、TC、TG、SBP、DBP及尿NAG与正常对照组比较均明显升高(P<0.05),而两组m-ALB、BUN及Scr差异无统计学意义(P>0.05)。经干预后,与模型组比较,黄连煎剂组大鼠体重、VFW、FINS、HOMA-IR、TC、TG、SBP、DBP均明显下降(P<0.05)。与盐酸小檗碱组、TUDCA组相比,黄连煎剂组大鼠FINS、HOMA-IR明显降低(P<0.05),但体重、TC、TG、SBP、DBP、尿NAG、BUN及Scr差异无统计学意义(P>0.05)。镜下观察显示,模型组大鼠肾脏肾小球肿胀明显,肾小球、肾小管毛细血管基底膜增厚,系膜区及间质增生,足突明显肿胀,大部分融合,肾小管上皮细胞严重空泡变性,有大量次级溶酶体形成,细胞器自噬,内质网及线粒体大部分消失。经黄连煎剂治疗后肾组织病理损伤明显减轻,但与盐酸小檗碱组、TUDCA组相比差异无统计学意义。结论黄连煎剂能够减轻体重,降低血脂和血压水平,并可改善胰岛素抵抗,且在改善胰岛素抵抗方面优于盐酸小檗碱、TUDCA。黄连煎剂还能减轻肾脏病理损害,具有保护肾脏的作用。Objective To investigate the protective effects of Coptis chinensis decoction on the metabolic index and renal function of rats with fructose-induced metabolism syndrome(MS). Methods From 56 male Wistar rats, 6 were randomly selected as normal control group, the other rats were daily fed with 10% fructose water for 8 weeks to reproduce the MS model. Twenty-four rats were established successfully as MS model and randomly divided into 4 groups(6 each): model group, Coptis decoction group, tauroursodeoxycholic acid(TUDCA) group and berberine hydrochloride(BH) group. The body weight, visceral fat weight, fasting blood glucose(FBG), fasting insulin(FINS), insulin resistance index(HOMA-IR), cholesterol(TC), triglyceride(TG), systolic blood pressure(SBP), diastolic blood pressure(DBP), urinary N-acetyl-β-D-glucosidase(NAG), urine microalbumin(m-ALB), blood urea nitrogen(BUN) and serum creatinine(Scr) levels were compared between the groups, and the renal pathological changes were observed by light microscopy and electron microscopy. Results The levels of body weight, visceral fat weight, FINS, HOMAIR, TC, TG, SBP, DBP and urinary NAG were obviously higher in the four MS model groups than in normal control group(P0.05), while the levels of m-ALB, BUN and Scr showed no significant difference(P0.05). After the intervention, the levels of body weight, visceral fat weight, FINS, HOMA-IR, TC, TG, SBP and DBP decreased significantly in Coptis decoction group than in model group(P〈0.05). Compared with BH group and TUDCA group, the levels of FINS and HOMA-IR decreased obviously in Coptis decoction group(P〈0.05), while the levels of body weight, TC, TG, SBP, DBP, urinary NAG, BUN and Scr showed no significant difference(P〉0.05).
