摘  要:目的:观察姜黄素治疗大鼠蛛网膜下腔出血后脑血管痉挛对神经功能的影响,为姜黄素治疗蛛网膜下腔出血提供理论依据。方法:30只SD大鼠按照数字随机表法分为空白组、对照组及治疗组,各10只。空白组大鼠注射0.3 mL生理盐水于枕大池,对照组注射相同体积自体血液于枕大池,治疗组在对照组基础上给予姜黄素。分别于术前、术后1 d、术后3 d、术后7 d、术后14 d固定尾巴后前肢的活动、四肢的自发运动、自发活动、攀爬动作、触须反应等进行神经功能评分;采用Morris水迷宫实验使大鼠进行游泳并学习寻找隐藏在水中平台测试对大鼠学习及记忆能力进行测定;并通过酶联免疫吸附试验(ELISA)测定大鼠海马组织肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)以及一氧化氮合酶(nitric oxide synthase,NOS)水平;实验结束时处死大鼠,运用PCR检测基质金属蛋白酶9(matrix metallo proteinase 9,MMP-9)信使mRNA表达水平。结果:空白组手术前后神经功能评分无显著差异,术前各组间无显著差异。术后第1天对照组与治疗组比较,差异无统计学意义(P>0.05);术后3 d、术后7 d、术后14 d治疗组经功能评分显著高于对照组,且均显著低于空白组(P<0.05);空白组逃避潜伏期显著低于对照组与治疗组,且治疗组显著低于对照组(P<0.05);空白组海马组织TNF-α、NOS水平显著低于对照组与治疗组,且治疗组显著低于对照组(P<0.05);空白组MMP-9 mRNA表达水平显著低于对照组与治疗组,且治疗组显著低于对照组(P<0.05)。结论:大鼠蛛网膜下腔出血容易发生脑血管痉挛,出现明显的神经功能障碍,采用姜黄素治疗后能够恢复神经功能,提高学习及记忆能力,抑制神经元的损伤、凋亡。Objective: To observe the effect of curcumin on neurological function of cerebral vasospasm after subarachnoid hemorrhage in rats to provide theoretical basis for curcumin treatment of subarachnoid hemorrhage. Methods: Thirty SD rats were randomly divided into blank group,control group and treatment group,with 10 rats in each group. The rats in the blank group were injected with 0. 3 mL of normal saline in the cistern magna and the control group was injected with the same volume of autologous blood in the cistern magna. The treatment group was injected with curcumin in addition to that of the control group. The activities of the forelimbs,the spontaneous movement of the limbs,the spontaneous activity,the climbing action and the tentacles response were examined and scored on the postoperative day 1,postoperative day 3,postoperative day 7 and postoperative day 14 with fixed tail.Morris water maze experiment was used to make the rats swimming and learning to find hidden platform in the water to test the memory ability of rats. The levels of tumor necrosis factor-α( TNF-α) and nitric oxide synthase( NOS) levels were measured by enzyme-linked immunosorbent assay( ELISA). The rats were sacrificed at the end of the experiment. The mRNA of matrix metallo-9( MMP-9) expression levels were measured with PCR assay. Results: There was no significant difference in neurological function scores between the two groups before and after operation. There was no significant difference between the two groups before operation. There was no significant difference between the control group and the treatment group on the first day after operation( P〈0. 05). The functional score of the treatment group was significantly higher than that of the control group at 7 days after operation and 14 days after operation( P〈0. 05).
