摘  要:目的:探讨糖化血红蛋白(HbA1c)、同型半胱氨酸(Hcy)和尿微量白蛋白(U-mAlb)联合检测2型糖尿病早期肾脏损伤的临床意义。方法:选取我院2014-12~2016-03收治的2型糖尿病(T2DM)患者160例为观察组(尿蛋白阴性患者84例和尿蛋白阳性患者76例),另选同期160例健康体检者为对照组。检测两组血清HbA1c、Hcy和尿U-mAlb的含量,同时对尿蛋白阴性患者标本行不同方法检测,记录检出阳性率并分析。结果:观察组各项指标阳性率均大于对照组,差异具有统计学意义(P<0.05);尿蛋白阳性患者较尿蛋白阴性患者3项指标均明显升高,差异具有统计学意义(P<0.05);尿蛋白阴性患者3项指标联合检测总阳性率为76.2%,明显高于U-mAlb单项检测的39.3%,差异具有统计学意义(P<0.05)。结论:糖化血红蛋白、同型半胱氨酸及尿微量白蛋白联合检测能大大提高2型糖尿病早期肾损伤的检测阳性率,值得临床推广。Objective:To explore the diagnosis value of joint detection of serum homocysteine (Hey), glycosylated hemoglobin (HbAlc) and microalbuminuria (U -- rnAlb) in type 2 diabetes mellitus (T2DM) with early renal damage. Methods: A total of 160 patients with T2DM (84 cases of urinary protein negative group and 76 cases of urinary protein positive group) during December 2014 to March 2016 were selected as observation group,and 160 cases of healthy people taking medical examination at same period were selected as control group.The content of HbAlc, Hcy and U--mAlb were detected. At the same time the urinary protein negative group were given different detection methods, the positive rate was recorded and anaylzed.Results:The positive rate of HbAlc, Hcy and U--mAlb in observation group were higher than those in control group, the difference were statistically significant (P〈0.05).Three indicators in urinary protein positive group were significantly higher than those in urinary protein negative group,the difference were statistically significant (P〈0.05).The total positiverate in urinary protein negative group with combined detection was 76.2 %, significantly higher than the 39.3% of detection of U--mAlb individual (P〈0.05).Conclusion:The combination of HbAlc,Hcy and U- mAlb can greatly improve the early renal damage detection rate of T2DM,worth clinical promotion.
