纵向单针套叠显微镜下输精管附睾管吻合术治疗梗阻性无精子症

摘　　要：目的观察纵向单针套叠显微镜下采用输精管附睾管吻合术治疗附睾梗阻性无精子症的临床疗效。方法选取2014年1月至2017年1月在本院就诊并确诊为附睾水平梗阻性无精子症的61例男性不育患者,均行纵向单针套叠显微镜下输精管附睾管吻合术。术后随访复通率、精液常规及自然受孕情况,随访时间为1年。术后任何一次精液常规提示有活动精子即定义为复通。结果共51例获随访,33例(64.7%)复通,11例(21.6%)配偶已受孕。结论纵向单针套叠显微镜下输精管附睾管吻合术可以很好地治疗附睾水平梗阻性无精子症,可在术后获得新鲜精液,实现自然受孕的可能。Objective To observe the clinical effect of longitudinal single needle intussusception microsurgical vasoepididymostomy in the treatment of epididymal obstructive azoospermia. Method From January 2014 to January 2017, we performed longitudinal single needle intussusception microsurgical vasoepididymostomy in 61 patients with epididymal obstructive azoospermia. After the operation, we followed up the rates of recanalization, semen routine and natural conception for one year. Any seminal routine indicates active spermatozoa is referred to as recanalization. Result We achieve the information of 51 patients. Of the 51 patients followed up for one year, motile sperm were found in 33 （64.7%）, spontaneous pregnancies were achieved in 11 （21.6%）. Conclusion Longitudinal single needle intussusception microsurgical vasoepididymostomy is an effective treatment of obstructive azoospermia,we can get motile sperm after the operation, and it provides a possibility of spontaneous pregnancies.
