异常糖链糖蛋白(TAP)在甲状腺乳头状癌中表达及临床意义

摘　　要：目的探讨异常糖链糖蛋白(TAP)在甲状腺乳头状癌(PTC)中的表达情况及临床意义。方法选取本院收治的PTC患者62例作为PTC组,术后病理证实为PTC,术后行内分泌治疗。选取同期良性甲状腺疾病患者57例、健康体检者50名分别作为良性组和正常对照组。分析三组TAP凝聚颗粒面积、表达阳性+临界型的几率及TAP诊断PTC的敏感性、特异性。了解PTC患者内分泌治疗效果对TAP的影响。结果 PTC组TAP凝聚颗粒面积大于良性组和正常对照组,TAP表达阳性+临界型的几率为83.87%,明显高于良性组的24.56%和正常对照组的6.0%,且良性组与正常对照组有差异,差异有统计学意义(P<0.001);TAP检测PTC的敏感度67.74%,特异度为64.49%。术后12个月影像学及甲状腺素检查证实无复发转移。PTC患者术后及内分泌治疗后TAP凝聚颗粒面积均小于治疗前,TAP阳性+临界型的几率均低于治疗前,差异有统计学意义(P<0.001),且PTC术后、内分泌治疗后与对照组比较差异无统计学意义(P>0.05)。结论 TAP可作为PTC筛查、疗效监测的重要指标。Objective To investigate the expression of tumor abnormal protein（TAP） in papillary thyroid cancer （PTC）and its clinical significance. Methods 62 cases of PTC patients in the hospital were selected as PTC group. Postoperative pathology confirmed PTC and endocrine therapy was performed after operation. Another 57 patients with benign thyroid diseases and 50 healthy subiects were selected as healthy group and normal control group at the same time. The TAP aggregation particle area, the probability of positive and critical type, sensitivity and specificity of TAP in the diagnosis of PTC in the three groups were analyzed. The effect of endocrine therapy on TAP in pa- tients with PTC was also analyzed. Results The TAP aggregation particle area was larger in PTC group than the be- nign group and normal control group. The probability of positive expression and critical type of TAP in PTC group （83.87 ~ ） was significantly higher than that in benign group （ 24.56 % ） and normal control group （ 6.0 % ）, and there were statistical significantly differences between benign group and normal control group（P%0. 001）. The sensitivity and specific ity of TAP in the detection of PTC were 67. 74% and 64.4g%. 12 months after operation,imaging and thyroxine detection confirmed that there was no recurrence or metastasis. After operation and endocrine therapy, TAP aggregation particle area and the probability of positive expression and critical type of TAP were smaller and lower than those before treatment（P〈0. 001）but there were no statistical significantly differences between OTC group and the control group（P〉0.05）. Conclusion TAP can be used as an important indicator for screening PTC and monitoring the curative effect.
