抗SSA抗体、抗SSB抗体、抗α-胞衬蛋白抗体联合检测在干燥综合征诊断中价值探讨

摘　　要：目的 本研究旨在通过联合检测干燥综合征中抗SSA抗体、抗SSB抗体、抗α-胞衬蛋白（Fodrin）抗体IgA和IgG的表达,探讨其在干燥综合征诊断中的应用价值.方法 选自2012年10月至2015年8月在我院就诊的患者及健康体检者,应用酶联免疫吸附试验（ELISA）定量检测血清中抗α-Fodrin IgA、IgG抗体,免疫印迹法检测抗SSA抗体、抗SSB抗体.共108例原发性干燥综合征（pSS）,61例继发干燥综合征（sSS）,对照组为98例正常健康体检者.结果 抗α-胞衬蛋白抗体IgA、抗SSA抗体和抗SSB抗体三种抗体联合检测诊断SS敏感性高于三种抗体单项检测的任一种（P〈0.05）,尤其诊断pSS.血清中抗α-Fodrin IgA和IgG抗体在干燥综合征中的阳性率分别为78.0％,40.8％,显著高于SLE、RA,差异有统计学意义（P〈0.05）.血清抗SSA抗体、抗SSB抗体均阴性的干燥综合征患者血清中抗α-胞衬蛋白抗体有20％以上阳性.抗α-Fodrin IgA、IgG抗体阳性的pSS患者病情活动相关指标高于阴性患者.结论 抗α-Fodrin抗体对干燥综合征的诊断有一定的意义,其分型检测更有利于干燥综合征的早期诊断.pSS患者中抗α-fodrin IgA抗体比IgG抗体的存在更常见.通过联合检测抗α-胞衬蛋白抗体、抗SSA抗体和抗SSB抗体可提高干燥综合征诊断的敏感性,对于传统抗体阴性的患者有一定的诊断价值.Objective To determine the IgA and IgG expressions of combined detection of SSA antibody,anti-SSB antibody,and anti-α-fodrin antibody in Sj？gren＇s syndrome,and to investigate their clinical value in the diagnosis of Sj？gren＇s syndrome. Methods Patients and healthy subjects,who were hospitalized in our hospital between October 2012 and August 2015,were included in the study. The anti-α-Fodrin IgA and IgG antibodies were determined by enzyme-linked immunosorbent assay（ELISA）. Anti-SSA antibody and anti-SSB antibody were detected by immunoblotting. There were a total of 108 cases of primary Sj？gren＇s syndrome（pSS）,61 secondary Sj？gren＇s syndrome（sSS）and 98 normal healthy subjects in the control group. Results （1）The SS sensitivity diagnosed by the combined detection of anti-α-lytic antibody IgA,anti-SSA antibody and anti-SSB antibody was higher than that of the separate detection（P〈0.05）, especially the diagnosis of pSS.（2）The positive rates of serum anti-α-Fodrin IgA and IgG antibodies in the Sjogren＇s syndrome were 78.0%and 40.8%,respectively,which were significantly higher than those of SLE and RA,and there were statistically significant differences（P〈0.05）.（3）The serum anti-α-lytic protein antibody was more than 20%positive in the serum of Sjogren＇s syndrome patients with negative serum anti-SSA antibody and anti-SSB antibody.（4）The disease-related indicators in patients with positive anti-α-Fodrin IgA and IgG antibodies were higher than those in the negative patients. Conclusion Anti-α-Fodrin antibody has certain significance in the diagnosis of Sjogren＇s syndrome,and its type detection is more favorable for the early diagnosis of Sjogren＇s syndrome. Anti-α-fodrin IgA antibody in pSS patients is more common than the IgG antibody. The combined detection of anti-α-fodrin antibody,anti-SSA antibody and anti-SSB antibody can improve the sensitivity of Sjogren＇s syndrome diagnosis,providing certain for t diagnostic value for conventional anti
