七情所伤的致病特点

摘　　要：目的研究七情致病的规律和特点,指导中医养生、预防和治疗。方法对50份精神障碍患者填写的提纲式作业进行质性分析研究。对问卷内容中情志导致症状的相关字段进行编码,并进行总结和分析。结果怒、思(怀念)、悲、恐四种情志可以导致症状,其中恐导致症状的频率最多;情志所导致的症状以失眠最为常见;不同情志所伤的脏腑没有显著差异(P<0.05);思(思维)不能单独导致症状但可以与其他情志协同导致症状。结论研究结果不支持"恐伤肾"等"本情志伤本脏"学说;情志致病主要涉及心、肝两脏,其次为脾,病机以郁、火、虚、风等为特点;思在七情当中具有中心地位;临床上面对症状尤其是失眠需要谨慎考虑是否由情志因素所导致。Objective To study the rules and specificities of emotion-induced diseases in order to direct TCM life-nurturance,prevention and treatments. Methods A qualitative analysis on 50 mental disorders patients outline homework was carried out. The excerpts related to emotion induced physical symptoms were coded and then summarized and analyzed. Results Four emotions,anger,longing,sadness and fear can induce symptoms,fear being the most prevalent,insomnia is the most common symptom induced by emotions,there is no significant difference（ P〈0. 05） between the organs-zang harmed by different emotions,thinking cannot induce symptoms on its own but can induce symptoms in common with other emotions. Conclusion The results didn＇t support the hypothesis of damage of an organ-zang by its related emotion like ＂kidneys damage by fear＂; heart and liver are the most related organs-zang implicated in emotion induced diseases,spleen being also implicated,and the specificities of the disease mechanisms are stagnation,fire,deficiency and wind. In clinic,attention has to be paid on the possibility of emotional factors of physical symptoms.
